- The Dementia-focused Health Coach provides support, coaching, and care coordination to individuals living with dementia and their caregivers. You will meet regularly with an interdisciplinary team for expert consultation and guidance.
- Ideal candidates have prior experience working with geriatric populations (either in healthcare or community organizations) and a passion for connecting with individuals living with dementia.
- This position is 2.5 days/week, daytime hours, weekdays only.
- Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education: High School Diploma preferred. Bachelor’s Degree preferred.
- License/Certifications: Valid Maine Driver’s License. Health Coach certification preferred.
- Experience: 2 years' working in a social service or health care setting or equivalent educational setting preferred.
- Demonstrated ability to work with individuals and families who have complex social and clinical needs preferred.
- Bilingual preferred.
- Sensitivity to cultural and ethnic diversity preferred.
- Local community resident with good knowledge of the resources available in the community preferred.
- Excellent organizational and critical thinking skills and ability to prioritize workload and to demonstrate flexible problem solving approach.
- Excellent communication and interpersonal skills and abilities to respond to diverse needs from patients, families and employees.
- Familiarity with medical terminology preferred. Ability to project a pleasant and professional image even under stressful situations. Ability to maintain confidentiality.
- Must complete most duties with minimum direct supervision and demonstrate ability to collaborate with teams of individuals in diverse settings.
- Ability to travel frequently to PCP practices and other sites where patients receive care. Proficiency in computer use, internet and health information technology; knowledge of EPIC electronic medical record system preferred.
