  • The Dementia-focused Health Coach provides support, coaching, and care coordination to individuals living with dementia and their caregivers. You will meet regularly with an interdisciplinary team for expert consultation and guidance. 
  • Ideal candidates have prior experience working with geriatric populations (either in healthcare or community organizations) and a passion for connecting with individuals living with dementia. 
  • This position is 2.5 days/week, daytime hours, weekdays only. 
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
  • Education: High School Diploma preferred. Bachelor’s Degree preferred.
  • License/Certifications: Valid Maine Driver’s License. Health Coach certification preferred.
  • Experience: 2 years' working in a social service or health care setting or equivalent educational setting preferred.
  • Demonstrated ability to work with individuals and families who have complex social and clinical needs preferred.
  • Bilingual preferred.
  • Sensitivity to cultural and ethnic diversity preferred.
  • Local community resident with good knowledge of the resources available in the community preferred.
  • Excellent organizational and critical thinking skills and ability to prioritize workload and to demonstrate flexible problem solving approach.
  • Excellent communication and interpersonal skills and abilities to respond to diverse needs from patients, families and employees.
  • Familiarity with medical terminology preferred. Ability to project a pleasant and professional image even under stressful situations. Ability to maintain confidentiality.
  • Must complete most duties with minimum direct supervision and demonstrate ability to collaborate with teams of individuals in diverse settings.
  • Ability to travel frequently to PCP practices and other sites where patients receive care. Proficiency in computer use, internet and health information technology; knowledge of EPIC electronic medical record system preferred.
