THE POSITION: The County of Ventura Ambulatory Care is currently recruiting registered nurses for our Enhanced Care Management (ECM) Team. ECM provides field-based case management services to members with highly complex medical, mental health, and substance use needs. ECM is a benefit through Gold Coast Health Plan that provides extra services to help members get the care needed to stay healthy. ECM team provides systematic coordination of care to ensure successful participation in medical treatment. Coordination includes communicating with various doctors and other providers involved in member’s medical care. ECM offers care management services by providing a whole person care approach to address social drivers of health.

This position will work a 9/80 schedule and will require travel throughout the County of Ventura to complete field-based assignments. Candidates with experience working with substance use, and/or the mentally ill are encouraged to apply.

What We Offer:

  • Educational Incentive for completion of: 2.5% for an Associate's degree, 3.5% for a Bachelor's degree, or 5% for a Master's degree.
  • Bilingual Incentive: Some positions may be eligible for bilingual incentive depending on the applicable memorandum of agreement and the needs of the department. In order to qualify for this incentive, incumbents in eligible positions must take and pass the applicable bilingual fluency exam.
  • Certification Pay: $0.813 per hour based on scheduled work week hours for each qualified certification up to a maximum of five (5) certifications. One of the five certifications may be paid at $2.00 per hour for a National Certification. List provided in the CNA MOA, Exhibit 9.
  • Continuous County Service Incentive Pay: 1% for 7 years, 1.25% for 12 years and 1.5% for 17 years
  • Flexible Credit Allowance: Biweekly contribution towards medical, dental, and/or vision insurance from authorized plans.
  • Pension Plan: Participation in the County's defined benefit plan.

APPROXIMATE SALARIES: 

Primary Care Clinic Registered Nurse I: $40.00 - $42.04 Hourly

Primary Care Clinic Registered Nurse II: $47.42 - $56.69 Hourly

AGENCY/DEPARTMENT: Health Care Agency – Ambulatory Care

Primary Care Clinic Registered Nurse I/II are represented by the California Nurses Association (CNA) and are eligible for overtime compensation.

The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-Term), Intermittent, Per Diem Pool, and Extra-Help vacancies within the Health Care Agency. There is currently one (1) Regular vacancy.

NOTE: If appointed at the lower level, incumbents may be promoted to the higher level without further examination upon meeting the minimum requirements, demonstrating satisfactory performance, and in accordance with the business needs of the department.

Examples Of Duties

Primary Care Clinic Registered Nurse I:

Duties may include but are not limited to the following:

  • Coordinates and facilitates delivery of care to patient and family in a primary care capacity, specifically in a field setting.
  • Assesses and identifies nursing care needs of patients and plans, implements, and evaluates interventions appropriate to these needs.
  • Actively participates in patient care team duties and activities, including pre-visit planning, identifying gaps in care, health maintenance needs, and team huddles.
  • Monitors clinical areas to minimize patient wait time and to address any potential acute situation. 
  • Prepares and operates mobile nursing station and provides a variety of care to patients throughout Ventura County. 
  • May support providers in medication management duties, including providing refills by protocol.
  • Provides training and clinical guidance to clinical support staff in a field setting.
  • Provides triage of patients with proper disposition and appropriate level of care using agency triage protocols.
  • Ensures appropriate documentation in the electronic medical record.
  • Coordinates patient care by following up on orders, including referrals to ancillary and specialty services. 
  • Maintains clean and safe patient areas, demonstrates adherence to and observes all safety policies and procedures, inclusive of infection control rules and regulations. 
  • Answers telephones and takes messages, reviews portal messages and routes as appropriate, and completes assigned tasks to ensure patient needs are addressed. 
  • May provide case management to patients with high risk/chronic conditions to support treatment plan and patient self- management goals.
  • Works with high volume of patients, internal/external customers, and appropriately manages frequent changes, delays or unexpected events.
  • Participates in assigned training and meetings.
  • Maintains and adheres to HIPAA.
  • Reports patient safety incidents to supervisor and via the patient safety reporting software.
  • Performs other related duties as required

Primary Care Clinic Registered Nurse II:

In addition to the duties above:

  • Develops and implements patient and family education programs;
  • Assists in establishing patient care standards and tracks the performance of nursing staff in the assigned area;
  • Participates in the development of clinic policies, procedures, and protocols for patient care.
  • Participates in nursing committee activities

Typical Qualifications

EDUCATION/TRAINING/EXPERIENCE:

Primary Care Clinic Registered Nurse I:

Requires no previous experience.

Primary Care Clinic Registered Nurse II:

Requires six (6) months of full-time professional registered nursing experience.

NECESSARY SPECIAL REQUIREMENTS:

  • Must possess and maintain a current, valid licensed as a Registered Nurse issued by the State of California
  • Must have a current, valid Basic Life Support (BLS/CPR) certification by the first day of employment
  • Must be willing and able to successfully complete the Health Care Agency Pharmacology examination within three (3) months of employment
  • Must possess and be able to maintain a California Driver's License
  • Must be willing and able to travel throughout Ventura County on work assignments. 

DESIRED

  • Experience in case management in a primary care setting.

Knowledge, Skills, and Abilities:

Working knowledge of: the principles, practices, and techniques used in nursing care and case management in primary and specialty clinics and in a field setting

Comprehensive knowledge of (Level II only): the principles, practices, and techniques used in nursing care at specialty clinics.

Working ability to: serve as a resource to others by providing patient education and appointment coordinator; work proficiently in emergency and/or stressful situations; maintain confidentiality of patient/client information; effectively maintain a positive working relationship with the medical staff, public, patients, and hospital administration; and communicate effectively, both orally and in writing.
