As an Advanced Nurse Practitioner within the Rapid Response Team of HCRG Care Group, you’ll join a team of support workers/HCAs, physiotherapists, occupational therapists and nurses who are focused on exceeding the needs of patients within our community of Dartford, Gravesend and Swanley localities. You will support the delivery of care to a range of complex patients which will include: life limiting conditions like diabetes, in dwelling long term catheter, urgent wound care etc, supporting rapid interventions for those patients reaching end of life and helping prevent any unnecessary hospital admissions to a patient in crisis. You will be based primarily in Dartford, Gravesend and Swanley with travel to Swale required occasionally.

As the Advanced Nurse Practitioner you will be working closely with a multidisciplinary team where you will use your experience to practice autonomously and flexibly. Practitioners should have significant experience in community nursing or rapid response service. Working within the nursing team, you will work with the service manager, and clinical lead to deliver robust clinical.
You will have an Advanced Nurse Practitioner qualification, NMC registered and hold a prescribing qualification.
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
