Job Description
We are seeking talented and team-oriented Qmap/Caregiver to join our Balfour – Louisville community. We are committed to a work environment of integrity, respect and a customer focus.
Schedule: Part-Time or Full-Time | 6:30am-2:30pm or 2:30pm-10:30pm
Pay for QMAP: $19-$22/hr
Pay for Caregiver: $18-$20/hr
$1.50 evening differential rate added after 6:30pm!
What will I do every day?
- Assist residents with simple requests as needed
- Supervise or assist with bathing, dressing, grooming, meals, ambulation and other activities
- Provide companionship and develop strong resident relations
- Respond to emergencies according to policies and procedures
- Document care provided to residents consistent
- Provide input to assure that resident individual service plans are current
- Review 24 hour report and care check lists regularly
- Pass meds
What will I need to be successful in this role?

- High school education or GED equivalent preferred
Special Requirements/Certifications I may need?
- First Aid certification and CPR, if required
- Must meet state training requirements and/or certifications
- Nursing Assistant Certification (CNA) if required
- QMAP certification is preferred
