Job Description:

Bellin Health is looking for a highly engaged and driven physical therapist or physical therapist assistant interested in a job with the opportunity to advance professionally. We are seeking someone who will contribute to our highly engaged culture driven to practice at the top of our scope of practice by utilizing the most evidence based care. The position is specifically targeting a professional with an aspiration for lifelong learning. The position will include working as a part of our Therapy Services department on Bellin’s hospital campus. Settings include acute care at Bellin Memorial Hospital and inpatient rehabilitation at Bellin’s 14 bed inpatient unit.

Performs and documents physical therapy evaluation, treatment, and discharge of patient according to established patient goals consistent with physician referrals. Communicates with patient, family, and members of health care team. Educates patient and public on health, wellness, and prevention of injury and illness as they pertain to physical therapy.

Responsible and accountable for independently providing quality professional physical therapy services to patients and their families and significant others adhering to the WI Physical Therapy Practice Act, APTA Guide for Professional Conduct and Code of Ethics and Bellin Health Policies and Procedures. The responsibility includes providing a safe environment for patients, staff, and visitors and working collaboratively with the multidisciplinary health care team to provide the best outcomes for the patient.

Qualifications:

Wisconsin Physical Therapist or Physical Therapist Assistant license required

CPR certification required
