Why does the job exist?

Multi-Level Services Family Support Advocate completes a comprehensive family assessment and safety/risk screening/assessment for families referred via CYFD Statewide Central Intake who have a screened in referral and meet the eligibility requirements of the Multi-Level Response program. These screened in reports will be diverted from investigations and to the multi-level response program.

Family Support Advocates complete assessments with families to identify family needs and barriers that led to referral, build working relationships with the family and community providers, refer families to needed supports and resources in the community, and support them in accessing needed services.

The Family Support Advocate reports data and interactions with families and community agencies in the agency database.

How does it get done?

Complete comprehensive family assessments with families who have a been referred from CYFD Statewide Central Intake via a screened in referral. Work with family to identify resources that have the capacity to increase family stability and support family in accessing needed services. Complete referrals to community-based supports and resources, to include a warm hand-off connection to resources. Update assessment plan as necessary based on needs identified in family assessments. 30%

Create an individualized family well-being plan collaboratively with the family, clearly identifying the community resources and supports that they family is referred to, as well as the responsibilities of the multi-level family support advocate and the family in connecting with the identified and agreed upon referrals. 30%

Conduct home visits with families who are receiving Multi-Level Response services to support connection to referral sources. Contact Statewide Central Intake (SCI) when there are safety concerns. 20%

Transport families as needed to identified support services 10%

Document results of family contacts and assessments in the identified case management system 5%

Work with other state agencies in continued implementation of Multi-Level Response across various sectors 5%

Ideal Candidate

Experience as it relates to:

Working directly with Children, Youth, and their families;

Partner with Community Service providers as it relates to Children, and Youth;

Working with Youth in a school setting;

Determining eligibility for services that might be needed for the Children, Youth and Families;

Conduct and complete assessments of safety, risks, strengths, and family needs;

Developing Safety plans for the Children, Youth and Families;

Participate in multidisciplinary meetings;

Identify, provide, and/or arrange timely assessments/evaluations/ interviews;

Conduct medical case management activities for eligible clients.

Minimum Qualification

Bachelor's degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, and/or Family Studies/Services and four (4) years of any combination of experience including working with communities, working on health or social service-related matters, social work/case management experience, behavioral health, and/or health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling eight (8) years may substitute for the required education and experience.

Employment Requirements

Must possess and maintain a valid Driver's License. Pre-employment background investigation is required and is conditional pending results.

Working Conditions

Office setting, exposure to Visual/Video Display terminal (VDT) and extensive personal computer and telephone usage with extended periods of sitting. May work under frequent time constraints. Significant travel statewide is required, approximately 25% of time.
