INCUMBENTS IN THIS POSITION WILL BE REQUIRED TO TRAVEL WITHIN THE STATE TO COMPLETE THEIR WORK ASSIGNMENTS.
Incumbent will be responsible for working with families who are either at risk for involvement in the child welfare system or have current involvement in the child welfare system and are at risk for out of home placement. The Family Navigator will be responsible for completing comprehensive family assessments, developing plans with the families to address their needs, building working relationships with families, supporting them to access needed services, and provide family preservation services if community-based services are not available in the community.
Conduct home visits with families who have been referred to Family Preservation services to identify appropriate service needs, identify family strengths, and support engagement in voluntary services. Contact Statewide Central Intake (SCI) when there are safety concerns. 20%
Complete comprehensive family assessments with families who are either at risk for involvement in the child welfare system or have current involvement in the child welfare system and are at risk for out of home placement. 15%
Develop a plan with families to meet the needs identified in the comprehensive family assessment. 15%
Provide case management and support services to families participating in Family Preservation services to include connection to community resources and/or provision of family preservation interventions if none are available in the community. 20%
Transport families as needed to identified support services. 10%
Work with other state agencies in continued implementation of CARA programming, including ongoing provision of services to CARA families currently receiving support from CYFD. 10%
Work with internal and external stakeholders to provide support for partners working with families who would benefit from family preservation services. 10%
Experience as it relates to:
Working directly with Children, Youth, and their families;
Partner with Community Service providers as it relates to Children, and Youth;
Working with Youth in a school setting;
Determining eligibility for services that might be needed for the Children, Youth and Families;
Conduct and complete assessments of safety, risks, strengths, and family needs;
Developing Safety plans for the Children, Youth and Families;
Participate in multidisciplinary meetings;
Identify, provide, and/or arrange timely assessments/evaluations/ interviews;
Conduct medical case management activities for eligible clients.
Bachelor's degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, and/or Family Studies/Services and four (4) years of any combination of experience including working with communities, working on health or social service related matters, social work/case management experience, behavioral health, and/or health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling eight (8) years may substitute for the required education and experience.

Must possess and maintain a valid Driver's License. Pre-employment background investigation is required and is conditional pending results.
Office setting, exposure to Visual/Video Display terminal (VDT) and extensive personal computer and telephone usage with extended periods of sitting. May work under frequent time constraints. Significant travel statewide is required, approximately 25% of time.
