The Social Worker for the Cancer Program performs Social Service duties for assigned patients in the program. Responsibilities include regular liaison with members of the treatment team, discharge planning, aftercare follow-up and liaison with community agencies as needed. The Social Worker conducts the psychosocial assessment for assigned patients. The Social Services Clinician provides support to the patients and their family systems. The Social Worker helps to coordinate and to provide family services. The Social Worker reports to the Manager of Oncology and Specialty Services.
