Job Description Summary

Under the direction of the System Executive Director of Case Management & Care Transitions, the System Manager of Ambulatory Social Work provides overall leadership in Social Work for the MUSC Health ambulatory settings.

  • The System Manager of Ambulatory Social Work is responsible for the development and implementation of cross-system strategic initiatives in various areas such as quality improvement, Social Work care delivery, programming, and training.
  • The System Manager of Ambulatory Social Work works collaboratively with inpatient and ambulatory leaders to address the myriad of system-wide Social Work workforce initiatives related to best practices, training/development, standard work and recruitment/retention.

Job Description

  • Shares accountability with other MUSC leaders for the daily monitoring of Social Work indicators and performance, identification and escalation of problems, and initiation and evaluation of action plans for achieving targets and improving the quality of care and services.
  • Oversees the coordination of Ambulatory Social Work programs and assures that Social Work programs within the system are realigned with the objectives of the system and meet the requirements of emerging developments.
  • Evaluates the provision of Social Work care delivery to ensure staff meet the patient’s needs and maximize patient self-determination.
  • Facilitates staff participation and participates in the Quality Outcomes Program and/or unit-based quality initiatives for the purpose of developing and monitoring exemplary clinical outcomes.
  • Collaborates with administrative, medical staff and professional staff directors in planning Social Work department participation, within the hospital system, and educational programs and in formulating policies and practices in accordance with the administrative systems of the Corporation.
  • Lead system-wide performance improvement Initiatives that improve the constellation of biopsychosocial-spiritual services available to and promote health equity for patients, their families and their communities.
  • Advises on Ambulatory Social Work challenges, participates in the planning of programs and in defining the Social Work role across Ambulatory services and programs.
  • Skill and proficiency in applying highly technical principles, concepts and techniques central to the Social Work profession in the coordination process and including all patients and specifically those identified with complex medical conditions.
  • Knowledge of systems, aftercare planning, and health equity
  • Fundamentals of hospital organization, revenue cycle, and managed care
  • Regulations and laws applicable to hospital operation
  • Must have excellent verbal and written skills as well as be able to operate a personal computer, word processing programs, fax machine, photocopier and multi-line telephone.

Additional Job Description

Education: LMSW- Licensed Medical Social Worker required.

Required experience: 5 years in a supervisory, consultative, and administrative capacity in a social work organization, health or medical care facility, or organization adhering to acceptable standards. Knowledge of administration with administrative and managerial skills and abilities.
