Stonebridge at Montgomery is seeking an experienced Wellness Director to join our team!

Stonebridge at Montgomery is a Springpoint Senior Living Community located in Skillman, NJ. We are seeking a full-time Wellness Director to join our team. At Springpoint, our Wellness Director is responsible and accountable for the direct delivery of nursing services in accordance with the Assisted Living Program regulations and established standards.

Responsibilities:

  • Conduct an assessment/health assessment on new residents upon or prior to admission.
  • Develop a resident general service/health services plan based on the resident’s assessed needs.
  • Periodically monitor and evaluate each resident’s physical and mental status for significant changes as per the Assisted Living Program regulations. Conduct family meetings as needed regarding changes in level of care.
  • Provide medications and treatments to residents as needed. Follow-up with pharmacy and consultant as needed.
  • Review and ensure completion of monthly wellness checks on all residents.
  • Notify physician, family and Director of Nursing about changes or concerns about resident’s condition.
  • Respond to emergency situations when in the facility and be available 24 hours, 7 days a week.
  • Train Certified Medication Aides as per guidelines, follow-up and continue with required med passes, in-service(s), delegations as needed.
  • Schedule with clinic and/or outside providers follow-up visits/appointments, tests, and consults.
  • Conduct staff education and in-services on an annual and as-needed basis.
  • Complete all required documentation in an accurate and timely manner as required by the Assisted Living Program regulations.
  • Ensure that all residents have annual physicals with MD certification for AL.

Education and Experience:

  • High School Diploma or equivalent required.
  • Must possess a current NJ Registered Nurse license.
  • BSN Degree preferred.
  • At least 2 years’ supervisory experience required.
  • Experience as an Assistant DON in Skilled Nursing a plus.
