Purpose: Provide integrated behavioral health services to address the psychological, emotional, and behavioral needs of patients served by the Organization.

Detail of Key Job Responsibility Areas 

1. Behavioral Health

  1. Complete initial assessment and determine patient diagnoses.
  2. Provide individual, family, and group therapy services as appropriate. 
  3. Work with medical providers and the interdisciplinary clinical team to review and integrate behavioral health services into treatment planning.
  4. Develop and periodically update treatment plans to include collaboration with the patient and significant others as appropriate.
  5. Document all treatment sessions within 72 business hours per Trillium policy.
  6. Participate in behavioral health clinical team meetings and clinical supervision to support ongoing development of competencies and skills.
  7. Develop discharge plans and coordinate transition planning for patients completing treatment.
  8. Provide mentoring and/or supervision of interns as needed.
  9. Collaborate with other Trillium programs/services to support identified patient needs.
  10. Develop outcomes and prepare regular reports on individual, group, and program achievements as assigned.

2. Supportive Services

  1. Provide therapeutic support and education to patients and/or significant others to promote understanding of mental health issues, symptoms and symptom management, and interventions. 
  2. Provide short term motivational counseling to patients who are not yet ready, willing, or able to participate in behavioral health services.

3. Miscellaneous

  1. Requires the ability and commitment to respect and support inclusiveness and diversity including but not limited to individuals of different backgrounds, cultures, races, ages, sexual orientations, gender identities or expressions, experiences, opinions, etc.
  2. Requires individual demonstration of commitment to the One Trillium values, behaviors and business impacts, and modeling them in the organization.
  3. Responsible for maintaining confidentiality and security of all patient/client/employee protected and proprietary information.
  4. Employees are accountable for meeting the performance standards of their departments and must participate as requested in compliance audits, process improvement and quality improvement plans. 
  5. Other duties as assigned.

Qualifications

Master’s degree in counseling, social work, psychology or a closely related field required. Professional licensure required (LCSW, LMHC, LMSW, LMFT); willing to consider candidates practicing with a permit while working towards licensure. Minimum of one year of experience providing individual, family and/or group behavioral health counseling preferred. Experience with trauma-informed care, co-occurring disorders, and LGBTQ+ populations is preferred. 

Physical Requirements

While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
