Community Health Services Network ("CHSN") is a group of primary care offices that are organized and operated exclusively to support children and their families in neighborhoods of our community. Our practices share the vision of providing quality medical care to children and families in the neighborhoods in which they live. We act as a liaison between the neighborhoods and Cincinnati Children’s. We focus on how to best meet the unique needs of all children in the neighborhoods of our community. CHSN has offices in Ohio, Indiana, and Kentucky.
About the job: We are recruiting a Social Worker to serve the CHSN Team in Ohio. This Social Worker will build relationships and partner with the practices, make resources available to families, conduct abuse and suicide screenings, and respond to practice concerns. Offices are located in Anderson, Kenwood, Liberty, Mason, and Springdale. Additionally, we have offices in Hughes High School, Taft High School, and South Avondale Elementary School. These primary care offices can see anyone, not just students at those schools.
Qualifications: To be considered, candidates must have their master’s degree in Social Work and be a Licensed Social Worker in the State of Ohio. Candidates with 2-5 years of social work experience are ideal. Past experience with community practices/networks is a plus.
Pay: $28.50-$40.63/hour ($59,280-$84,510 annually)
