Position Summary:

Reporting to the Nursing Director, this position is responsible for planning, directing, coordinating, and providing high quality, individualized patient care using the components of the nursing process. Will be trained Post-Partum and Well-Baby experience.

Position Accountabilities:

The following are general job accountabilities:

1. Maintains current professional licenses and/or certifications as and when required by state, regulatory agency, or hospital mandate.

2. Demonstrates knowledge of, and adheres to, all applicable professional/regulatory practice acts, state/ federal regulations, and policies and procedures of San Ramon Regional Medical Center (SRRMC) and TENET CORPORATION.

3. Actively supports the organization’s mission, vision, values and goals. Participates in activities within the organization that support performance improvement. Participates in unit GEMBA initiatives and Tenet performance initiatives.

4. Demonstrates the knowledge and skills to safely and appropriately fulfill job specific responsibilities related to safety, security, emergency preparedness, control of hazardous materials/waste, life safety, medical equipment and utility systems as outlined in the hospital’s environment of care plan.

5. Completes mandatory TENET ethics program. Conducts all hospital business within the highest standard of ethical behavior.

6. Maintains required applicable education and in-service requirements and participates in departmental staff meetings (through attendance or reading meeting minutes).

7. Assures that the rights of all patients are respected and maintained by allowing for privacy, confidentiality, and dignity in the provision of service.

8. Determines the presence and type of Advance Directive, as appropriate. Contacts appropriate resources to assist the patient and family as needed.

9. Initiates a multidisciplinary, goal oriented care plan for each patient in collaboration with members of the health care team and provides care based on that plan. Updates the care plan each shift and closes care plan prior to discharge.

10. Adheres to SRRMC practices for pain management with specific consideration of patient’s

functional/developmental age, gender, cultural and/or personal preferences. Timely follows up with pain reassessment after interventions.

11. Barcode scanning compliance of 90% for NICU/L&D RN’s and 95% for pp/wb RN’s.

12. Provides appropriate basic nursing interventions. Competent in all routine nursing skills, including: (1) medication administration, (2) intravenous therapy (3) management of indwelling catheters and drains, management of incision/staple removal and (4) safe use of any medical equipment, such as glucose meters, IV pumps, PCA pumps.

13. Provides compassionate and caring support to patients and families.

14. Completes accurate and timely documentation (every 2 hours) of all nursing care provided to patients, including an evaluation of patients’ response to medications, treatments and procedures.

15. Accesses facility information systems to retrieve and utilize pertinent information for the provision of patient care. Accesses facility information systems in the performance of job related responsibilities. Documents thoroughly and completely in the electronic medical record (EMR).

16. Patient care handoff to next provider of care (nurse or physician) to include SBAR report, bedside report to include patient/family as able.

17. Follows Medical Center’s policy and procedure as they pertain to all hospital information systems.

18. Practices Standard Precautions while providing patient care and performing hospital services.

19. Timely communicates and follows the organization’s chain of command for notification of patient care and/or service issues.

20. Expresses and accepts feedback in a professional manner.

21. Demonstrates the ability to organize workload, set priorities, and use time efficiently to complete work on time and in an organized manner. Uses downtime to complete requisite .edu’s stocking rooms, organizing work area.

22. Delegates patient care tasks to ancillary personnel. And evaluates effectiveness of request/care.

23. Protects hospital resources through appropriate use of supplies and equipment.

24. Demonstrates reliability as evidenced by attendance records and punctuality.

25. Consistently attired appropriately for the professional environment. Includes changing to hospital provided scrubs for c-sections.

26. Demonstrates responsible role in noise reduction efforts in the department.

27. Assists in maintaining traffic control in the department.

28. Completes .edu’s and other required hospital education by the due date.

29. Adheres to SRRMC Behavior Standards, treating others respectfully.

30. Complies with San Ramon Regional Medical Center Policies/Procedures protecting patient information and the confidentiality (HIPPA) of the information in accordance with the Federal and State regulations.

31. Other related duties as assigned.

MOTHER/BABY & GYN RN unit-specific accountabilities:

1. Performs duties in a manner that provides for the psychosocial and spiritual needs of the patient and family.

2. Completes assessments, patient documentation and implements plan of care for patients in a timely manner.

3. Implementing patient care and safety measures with specific consideration of patient’s functional and developmental age; the care of the neonate (0 to 30 days), adolescent (12 to 17 years) and adult (18 to 70 years).

4. Assesses and manages pain effectively in the mother, neonate, and GYN patients (for example, providing post-op, post delivery, and post procedure care).

1. Assesses newborn pain using NIPS scoring, Manages pain effectively in the neonate (for example, using sucrose during circumcisions or Tylenol as ordered.

5. Ensures a safe patient environment by: providing information regarding safe infant handling, providing family education related to infant security measures, responding to infant security alarms and alarms monitoring patient care.

6. Performs routine well baby, postpartum and post-op care.

7. Provides mother/baby nursing care to include: immediate/subsequent skin to skin, early initiation of breastfeeding, assessment of maternal/newborn bonding, education on breastfeeding techniques, normal physiological changes in the postpartum mother and normal newborn.

8. Performs newborn screening exams (NBS, TcB, I.H.S,CCHD, Car Seat Tolerance Testing) prior to discharge.

9. Demonstrates the ability to provide care in emergencies and follows established emergency procedures and practices. Recognizes and responds appropriately to distress in the neonatal, maternal, and GYN patients

SPECIAL CARE NURSERY unit specific accountabilities:

1. Provides appropriate nursing interventions to include (1) gestational age assessment, (2) congenital defect screening, (3) mechanical suctioning, (4) tactile stimulation, (5) gavage and NG feedings, (6) IV placement, and (7) assisting physicians with procedures as required, (8) Oxygenation modalities NCPAP, HHFNC, intubation, hood and nasal canula (9) Umbilical line placement and management.

2. Attendance and management of Newborn Deliveries and Stabilization of High Risk Newborns and transfer to into SCN

Position Qualifications:

Minimum Education:

  • Graduate of an accredited school of Nursing;
  • BSN preferred.

Minimum Experience/ Skills:

  • 1 year recent acute care experience in NICU required.

Licenses/Certificates/Credentials:

  • Current California RN license
  • Current BLS certification
  • Current NRP certification
  • STABLE certification

Pay range: $75.61 - $92.01

Individual wages are determined based upon a number of factors including, but not limited to, an individual’s qualifications and experience

Education:

Required: Graduated from a state approved school of nursing or an accredited school.

Preferred: Bachelor's or Master's degree in Nursing

Experience:

Preferred: Recent acute care experience.

Certifications:

Required: Currently licensed, certified, or registered to practice profession as required by law, regulation in state of practice or policy; CPR. AHA BLS, and if applicable by corporate policy for unit of hire, AHA ACLS and/or PALS and/or NRP.

Physical Demands:

The RN provides high-quality, safe, cost-effective, total nursing care to all patients. The RN assumes responsibility for overall coordination and integration of patient care based on the nursing process. The RN directs care according to established standards of safety, risk management, QI, and Infection Control, with the goal being to discharge the patient with an optimum level of care provided. Assumes responsibility for coordination and planning of patient care within the prescribed medical regime. Administers all medications safely and in a timely manner per hospital policy. Performs and fulfills all HICS (hospital incident command system) responsibilities, assignments, tasks, and roles as directed and needed by the facility in times of emergency and/or disaster. Responsible for knowing the Hospital’s emergency codes, including the purpose and appropriate action for each, and adheres to the Hospital policies and procedures for each emergency code.
