A Brief Overview
The Patient Care Coordinator functions as a member of the patient care team. The position performs clinical tasks and procedures to support the delivery of care and to promote efficient use of the physician's time as well as performs all clerical duties required to complete patient Check-in/Check-out. The Patient Care Coordinator is responsible for assisting patients obtain services in a timely manner and assuring efficient utilization of the physician's appointment schedule and in the coordination of patient care. This position administers and coordinates referrals and service authorization process, rooming duties and performs other clinic front desk activities. Patient Care Coordinator has five levels that are distinguished from each other primarily by: 1. Entry: The complexity of assigned duties and responsibilities. 2. Proficient: Knowledge level of required clinical care skills. 3. Full Proficiency: Required level of business communications skills. (Department must have business need to progress) 4. Expert: Independence of action (degree of supervision received). (Department must have business need to progress) 5. Lead: Independence of action (degree of supervision received). (Department must have business need to progress) The Patient Care Coordinator I is an entry-level position with limited or no prior experience; learns to use medical terminology and concepts to resolve problems of limited scope and complexity; works on developmental assignments that are initially routine in nature, requiring limited judgment and decision making. Performs the more routine support work and in a learning capacity, assists in: - Patient Rooming - Vital Sings - Prepare Medical Records - Patient Phone Calls - Stocking of clinic area - Medication and supply ordering Core: - Rooming - Check-in/out - Vaccine administration - Medication Administration - POC testing - Specimen collection - Procedure assist - Prescription refills - CRM/In-Basket Mgmt Other duties as assigned: Specific Protocols as assigned Disability paperwork EverReady Checklist

Base Pay Scale: Generally starting at $30.61 - $34.45 per hour
The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
