Job Description

SCOPE OF POSITION

Community Health Navigators will provide hands-on assistance to address identified unmet health-related social needs. Individuals will be identified within target populations and assigned to a Navigator who will provide hands-on assistance to identify and meet social needs. Navigators will screen individuals for both social and medical needs including but not limited to food insecurity, utility needs, housing instability/quality, interpersonal violence, and transportation needs. This position will work hand-in-hand with other care team members to implement the program and help individuals address their social needs with community service organizations and resources that exist in the local area. The Navigator will conduct personal interviews with individuals, conduct screenings and will create and implement action plans with each individual assigned to their case load. All interactions will be recorded and reported within a documentation system.

The Navigator will provide in-depth assessment and care coordination follow-up with the individuals they are serving until their needs are resolved or their defined engagement period ends. In addition, the Navigator will document and track individual level navigation activities and outcomes including encounters, contact attempts and preparation of person-centered action plans. The Navigator will collect data and report said data. Navigators will create action plans, perform screenings, and document all progress and contacts. In addition, the Navigators will use a Community Referral Software platform to make referrals to needed resources.

Navigators must be willing make home visits, be out in the community and offer a personal touch to the individuals served to win trust ad successfully obtain needed services. It is preferrable if the Navigator is a long-time resident of the community they will serve. Prior experience as an outreach worker is a plus.

Navigators shall have the skills and experience to understand what low-income patients are going through and help them with difficult times.

EDUCATION AND EXPERIENCE

Bachelor’s degree in healthcare-related field such as social work or public health preferred, or an Associate’s degree with four years’ experience preferred, neither are required. In lieu of a degree, individual must have experience in working with individuals and communities to assist in linking to services, supporting resource attainment, and addressing health equity and social determinant insecurities. Experience in healthcare is preferred, but not required.

For those individuals working with the Recovery Program, the individuals must meet the qualifications as a Certified Peer Recovery Specialists (CPRS). Navigators must have a working knowledge of the drug and alcohol treatment system and demonstrate a commitment to the recovery community. This individual will promote self-determination, personal responsibility, and empowerment to those they are serving.
