Responsibilities

Terros Health is hiring a Clinicial Intake Specialist in Phoenix, AZ. 

Location: East. McDowell

Position Summary: Under Terros’ Health’s policies and professional requirements, the Clinical Intake Specialist participates as a member of an interdisciplinary clinical team working with individuals diagnosed with General Mental Health and Substance Use diagnoses. Works in conjunction with the intake clinician to complete the initial intake requirements, assigns patients to appropriate treatment groups and other services, completes demographics, assists in scheduling follow up appointments, completes administrative paperwork to ensure patients are placed into treatment in a timely manner, ensures intake documentation meets quality standards, and conducts outreach and engagement assistance. Communicates and documents patient progress and all outreach efforts in a professional, efficient, and timely manner. Uses brief clinical interventions, Motivational Interviewing, and supportive listening skills to engage and support patients receiving services. Works under the direct supervision of Health Center Director or Clinical Site Manager.

Essential Duties:

  • Works collaboratively with the intake clinician to complete the necessary case management/ administrative aspects of an intake to ensure patients are placed in treatment in a timely manner.
  • Completes patient demographics and ancillary requirements, including program location, program detail, patient provider, support services, release of information, state demographics, group referral information, PCP communication document, at risk crisis plan, scheduling medical appointments, and documenting services provided.
  • Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patients receiving behavioral health and/or substance use treatment services.
  • Provides excellent customer service to patients, internal and external provider agencies, State entities, or other stakeholders (e.g. probation officers, advocates, family members, hospital staff, jail staff, vocational rehabilitation, etc.) as needed.
  • Conducts face to face, telehealth and telephonic interactions with patients, family or other involved parties for the purpose of assessing, maintaining, or enhancing a person’s functioning.

Qualifications

  • A bachelor’s degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
  • A bachelor’s degree in the field of behavioral health and any behavioral health experience, or INDUSTRY: Behavioral health.
  • Associates degree in Medical or Behavioral Health field; or
  • High School diploma/GED equivalency and 3 years of behavioral health experience.
  • Basic understanding of the DSM-V preferred.
  • High level of competency in Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.
  • Excellent communication skills, evidenced by a non-judgmental approach and effective listening
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
  • Certified Case Manager preferred but not required.
  • Must have valid Arizona driver’s license, be 21 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test.
