Horizon Health, in partnership with Livingston Regional Hospital in Livingston, TN has an exciting opportunity for a Discharge Planner for our Inpatient Behavioral Health Program.
The Discharge Planner develops and implements all aspects of the discharge plan for each patient, demonstrates knowledge of regulations influencing discharge plans, and post-hospital care and participates in data collection efforts for quality assurance and quality improvement purposes.

Requires a Bachelor’s Degree in a behavioral health related field, and a minimum of 1-3 years of experience in discharge planning. Clinical Licensure highly preferred (LCSW, RN, LPC, etc.)
