JOB SUMMARY

The HIV Medical Case Manager at Rainbow Health is responsible for client need assessments, care coordination, risk reduction education, client advocacy, and HIV disease and medical care education. The ideal candidate will be comfortable having direct and honest discussions with clients regarding sensitive topics such as sexual activity and drug use behavior. Though this is not a mental health-based position, case managers may need to occasionally work with clients who are experiencing a crisis. Our case management program works from client-centered, strength-based, harm-reduction, and trauma-informed perspectives; comfort working from these perspectives is required for the success of our clients.

For nearly 40 years Rainbow Health has been advocating for and serving the LGBTQ+ community, those living with HIV, and all folks facing barriers to equitable healthcare. Since our formation through the Minnesota AIDS Project and Rainbow Health Initiative merger—and later the acquisition of Training to Serve—we have continued to grow and identify new ways to commit to our mission.

ESSENTIAL FUNCTIONS

  • Engaging with clients one-on-one to develop rapport and build relationships to respond to needs and engage in proactive client work with a caseload of up to 45-50 clients.
  • Completing formal and informal assessments with individual clients to identify client strengths and barriers to meeting needs, with an emphasis on needs connected to managing HIV health
  • Developing service plans in partnership with clearly defined goals and action steps client and a case manager will take to achieve identified goals
  • Meeting with clients in provider office and community settings including medical offices, public libraries, or other community gathering spaces as needed.
  • Having the desire to learn and maintain a thorough understanding of HIV disease, transmission, and services and delivering information to clients in appropriate ways to support client health management and transmission risk reduction and an understanding of program eligibility criteria, verification processes and documentation standards, and complete documentation using electronic databases
  • Developing and maintaining a thorough understanding of the intersection of chemical and mental health concerns and health management
  • Completing narrative written documentation of client interactions using professional and objective language
  • Collaborating with internal agency and external community HIV professionals and stakeholders.
  • Supporting team members in work efforts through client consultation, coverage, referral education, etc.
  • Other duties as needed to best support the agency and our clients

QUALIFICATIONS

  • High school diploma.
  • 2+ years of customer service or client focused background in housing, HIV, substance abuse, and/or mental health.
  • Access to reliable transportation and the ability to travel independently throughout the state of Minnesota is preferred.
  • Bilingual candidates are strongly preferred.
  • Individuals with lived experience or a passion towards our target populations (Gay/Bi/MSM/Queer, Transgender, African American, multi-racial communities that are disproportionately impacted by HIV, HIV positive people, and those engaging in risk behaviors for HIV in individual and group settings) are encouraged to apply.

WORK ENVIRONMENT

  • Work is currently being with a hybrid format, splitting time between the community and working from home. The person hired for this position will need to be comfortable meeting people within the community, participating in community events, and attending in-person meetings.
  • Employees working in person with clients or out of our offices are required to show proof of vaccination and attest to their vaccination status upon hire. Employees who are not vaccinated will be required to show a negative COVID-19 PCR test result once a week if working with clients or in our offices. This position is part of a bargaining unit represented by SEIU Healthcare Minnesota & Iowa. All wages, benefits, and other terms and conditions of employment will be established through negotiations with the union.
