The SHAPE-DP "Strategic Health Approach to Promote Health Equity and Diabetes Prevention” is a targeted expansion of Trinity Health's existing National Diabetes Prevention Program (DPP) efforts. The purpose is to advance health equity and reduce health disparities for people at risk for type 2 diabetes by implementing a bold strategy that promotes optimal health for people experiencing poverty and other vulnerabilities, including addressing unmet health-related social needs, by building a network of National Diabetes Prevention Program (NDPP) providers and payers across the Trinity Health service area.

This position leads the evaluation, including design, data collection, analysis, interpretation and visualization of results of Trinity Health’s approach to scaling DPP and addressing health related social needs. Applies statistical techniques and quantitative methods in reviewing data to develop scorecards, benchmarking studies, Centers for Disease Control (CDC) evaluations or integrated ad hoc analyses. Provides insights for strategic initiatives to the SHAPE-DP Director and teams across the healthcare system.

Operates in accordance with national standards and supports diabetes prevention initiatives aligned with program goals. Uses analytical and statistical skill set to research and make recommendations on business problems and to develop and implement data driven strategies to support key initiatives.

Serves as a subject matter expert by providing analytic consultation in collaboration with other team members (Director, Senior Population Health Equity Consultant, RHM delivery sites) and National contracted partners in identifying analytics options, opportunities and providing problem solving solutions in support of the project goals.

Leads the annual/bi-annual CDC evaluations such as the recipient evaluation and performance measures plan, Annual Performance Review (APR) and the data management plan.

Leads the CDC designed National Evaluation plan and case studies as assigned by the CDC. 

Evaluates approaches, efficiency, effectiveness, sustainability, and impact of the annual workplan goals on the short term, intermediate and long-term performance measures. Uses evaluation findings to conduct continuous quality improvement using established methods.

Collaborates with national partners and internal delivery sites to identify best practices and leads the ideation of presentations to publish/present findings at various public health or hospital convenings. Collaborates with other SHAPE-DP team members to publish policy briefs, infographics and manuscript work to ensure extensive dissemination of outcomes.

Responsible for validity and reproducibility of analyses. Maintains routine reporting and ensures completeness and accuracy of reports. Supports the design and development of datasets and dashboards from multiple data sources.

Researches measure definitions, algorithms, published journal articles and statistical methods to make appropriate recommendations to support project goals.

Develops queries to source high-volume, high-dimensionality data from multiple complex databases. Cleans and manipulates data as appropriate for report generation, dashboard creation, or simple to moderately complex statistical analysis.

Regularly monitors performance of many key performance indicators (including but not limited to: enrollment, retention, participant outcomes, social needs screening and interventions), highlights trends, examines interactions in metrics, and identifies opportunities for improvements to leaders and stakeholders. Shares analyses with Director to ensure that DPP delivery organizations are performing at sufficient levels to achieve outcomes within stated timeframes.

Develops portions of presentations, publications and applications in collaboration with other stakeholders. 

Draws strategic and tactical conclusions from data analyses through summarization, interpretation, and visualization of data so to make recommendations to leadership in an easy-to-understand communication format.

Builds strong relationships both internally and with external contracted partners and collaborates effectively on cross-functional teams. Collaborates with SHAPE-DP team to develop integrated ad hoc analyses aligned with program goals and grant deliverables.

Develops trusted relationships through efficient, high-quality execution of analyses and effective communication regarding expectations and timelines.

Other duties as needed and assigned by the Director of Health Promotion and Disease Prevention and CDC reporting requirements.
