Eddy SeniorCare is an exciting, innovative model of care for seniors, located both at 385 Watervliet-Shaker Road, Latham. We provide a team-based model of care including a physician, RNs, LPNs, Home Health Aides, physical/occupational therapists, social workers, transportation, a day center team, and more!

We are privileged to serve patients who are 55 years of age and older, who are living with a variety of chronic conditions requiring the support of a comprehensive program like ours to help them delay/avoid nursing home placement and remain living in the community as long as possible.

Eddy SeniorCare has a rewarding opportunity for full-time Physical Therapist responsible to collaborate with our interdisciplinary team to manage a caseload of patients requiring physical therapy, both restorative and maintenance.

What you will do:

• Work as a part of a team to manage a caseload of patients requiring home-based therapy, including a mix of those who require post-acute or subacute rehab as well as those benefitting from maintenance therapy.
• Promotes/provides physical therapy services in the home to assist the patient in achieving his/her optimal level of functioning. 
• Evaluates and treats individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
• Work Schedule: Monday-Friday, generally 8am-4:30pm

What you will need:

• NYS PT Licensure
• 1 year of experience.
• NYS Driver’s License; good driving record.
• Must possess a passion for caring for seniors and demonstrate a respectful, caring, professional dimeanor
• Strong ability to assess priorities
• High level of interpersonal skills to establish and maintain relationships with patients, families, and coworkers
• Excellent customer service skills

Pay Range: $37.60-$47.60

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.”
