Description:

Trinity Health Mid-Atlantic is seeking an experienced Director of Quality and Population Health to join our THMA Medical Group team!

Schedule: Hybrid Schedule, Full-time Days (Monday - Friday)

Under the direction of the THMA Medical Group Leadership, this position is responsible for the development, management and evaluation of strategies, standards, programs and practices addressing quality, patient safety, and delivery of patient care for THMA MG in accordance with the overall business objectives, and applicable third payer and accreditation requirements. The Director of Quality and Population Health will work closely with leadership within THMA to support strategies to improve the health outcomes of a group of individuals for which THMA MG or THMA CIN has accountability. The Director has responsibilities ranging from Quality initiatives, coordination with al clinics in THMA MG, development of relationships with internal and external customers, department financial management, and THMA MG policies and procedures. The accomplished Director will have the ability to develop strong partnerships with carriers and THMA CIN and participates in strategic planning to support the strategic objectives of the organization to drive operational excellence.

Responsibilities include, but not limited to:

  • Responsible and accountable for leadership and operational direction and excellence including achievement of the quality metrics related to the Trinity Health Medical Group Balanced Scorecard for THMA MG.
  • Oversees the development and implementation of the THMA MG population health/quality team
  • Meeting or exceeding established financial objectives
  • Facilitating committees and communication with key stakeholders
  • Responsible for maintaining relationships with internal and external customers.
  • Understand the quality and utilization metrics present in contracts and be able to educate staff on those metrics and participate and ensure reports on those metrics going out to providers, in collaboration with the THMA CIN.
  • Provides leadership and education on service excellence and acts as THMA MG liaison for patient experience and reputation management vendors.
  • Initiates organizational audits and make recommendations to management; plan, supervise, and review quality initiatives.
  • Develop departmental goals and work plans to meet organizational strategic initiatives.
  • Develop productive and positive relationships with providers, clinic personnel, administrative staff, business associates and the general public.

Minimum Qualifications:

  • Master's Degree in Nursing, Public Health, Health Care Administration or other health related field preferred; Bachelor's Degree required.
  • At least 3-5 years' management/director level experience required, preferably in the ambulatory setting
  • Licensed Healthcare professional such as RN, CMA preferred. 
  • Five to ten years professional experience in Healthcare Quality Management and demonstrated expertise in data analysis.
