We are actively searching for an expert clinician to help lead our team.
This position requires an expert clinical resource that mentors colleagues in all aspects of patient care delivery, and provides ongoing professional development of others.
Provides day-to-day leadership to align patient and family needs with resources within the department and organization through collegial collaboration with the guidance of the unit director.
Works directly with physicians and leads collaborative patient care rounds.
Initiates and implements quality improvement projects to further patient care.
Works hand and hand with unit director and clinical nurse specialist to assist new staff in onboarding, payroll and scheduling tasks.
Primary hours will be Monday-Friday, some weekend shifts may be required.
Consistent with the American Nurses Association Scope and Standards of Practice (2004), the Patient Care Coordinator is a professional nurse and a member of the Mercy Medical Center healthcare team and is accountable for the delivery of holistic, compassionate, individualized, and effective patient care demonstrated by the use of the nursing process: assessment, diagnosis, outcome identification, planning, implementation, and evaluation. In addition, the Patient Care Coordinator is responsible for behaviors consistent with the professional nursing role, appropriate to their education and experience, including activities related to quality of care, performance appraisal, education, collegiality, ethics, collaboration, research and evidence-based practice, and resource utilization. The Patient Care Coordinator facilitates interdisciplinary team planning to maximize patient outcomes throughout the continuum of care and serves as a clinical resource for the defined patient population. The Patient Care Coordinator assumes accountability for the nursing department in the absence of the Director.
POSITION EXPECTATIONS:

Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information (i.e. HIPAA, Stark, etc.) Practice within the legal scope of a registered professional nurse as outlined in the Iowa Board of Nursing Practice Act (2000) and retain accountability for one’s own nursing practice. Utilize research and evidence-based practice standards in the delivery of patient care. Continuously seek out opportunities to improve the quality of patient care delivery through innovative and creative problem solving. Support and participate in shared leadership and interdisciplinary decision-making. Demonstrate ethical conduct and practice consistent with the Code of Ethics for Nurses with Interpretive Statements (2001). Contribute to the positive image of nursing and enhance the magnetic culture of the organization by modeling professional behavior both in the organization and in the community. Demonstrate characteristics of practice (caring, collaboration, & clinical decision making) consistent with the RN’s established level of knowledge and experience on the Novice to Expert continuum (Benner, 1996) – see attached description of practice level. Serve as a teacher, mentor, and coach to patients, students, new nurses, peers, other colleagues, and staff. Embrace change and lifelong learning, taking personal accountability to seek out experiences that reflect current practice in order to maintain skills and competence in clinical practice and role performance. Seek feedback concerning one’s own practice.
REPORTING RELATIONSHIP:
Mercy Medical Center supports a shared leadership philosophy and encourages nursing staff to demonstrate accountability for their own practice. Therefore, the nursing organization reflects a flat structure. The Patient Care Coordinator reports to the Director, who in turn reports to the Vice President of Patient Care Services/Chief Nursing Officer. No subordinates formally report to this position, but the incumbent may serve as a mentor to other nursing staff and may delegate activities to unlicensed assistive personnel as outlined in the Iowa Nursing Practice Act and the Mercy Delegation Policy.
PATIENT CARE COORDINATOR DUTIES AND RESPONSIBILITIES:
The Patient Care Coordinator will adhere to the ANA Standards of Practice (2004):
Standard 1. Assessment The Patient Care Coordinator collects comprehensive data pertinent to the patient’s health or the situation.
Standard 2. Diagnosis The Patient Care Coordinator analyzes the assessment data to determine the diagnoses or issues.
Standard 3. Outcomes Identification The Patient Care Coordinator identifies expected outcomes for a plan individualized to the patient or the situation.
Standard 4. Planning The Patient Care Coordinator develops a plan that prescribes strategies and alternatives to attain expected outcomes.
Standard 5. Implementation The Patient Care Coordinator implements the identified plan.

WORKING CONDITIONS AND PHYSICAL DEMANDS:
In an 8-hour day, the employee must sit for 2 hours, stand for 2 hours, and walk for 4 hours. Employee’s job requires occasional (5-10% of work time) squatting, crawling, crouching, kneeling, balancing, frequent bending/stooping, and reaching above shoulder level, and pushing/pulling. Job requires the use of hands for simple and firm grasping and fine manipulating. Job requires lifting and carrying frequent (21-50% of work time) 11-24 lbs., and occasionally (6-10% of work time) up to 35 lbs. without assistance. May be exposed to marked changed in temperature and humidity, infectious diseases, hazardous waste, and chemicals. Job requires high level of mental concentration to attain established deadlines. Working environment includes frequent interruptions. Must be able to see at a near visual acuity level. Must be able to speak. Must be able to hear speech.
MINIMUM EDUCATION, LICENSURE, CERTIFICATION, AND EXPERIENCE REQUIRED:
The Patient Care Coordinator must be a registered nurse and must hold an active license to practice nursing in the state of Iowa. A baccalaureate degree in nursing and certification by a nationally recognized nursing organization is preferred and strongly encouraged. An applicant must have a BSN degree in nursing. The registered professional nurse must continuously seek opportunities to enhance their own professional nursing practice. Membership in an appropriate professional nursing organization is recommended and previous leadership experience including charge experience, community leadership, group facilitation, committee or project involvement or an active role in the change process is necessary. The job requires a high degree of interpersonal skills to deal with a range of complicated problem situations. Child and Dependent Adult Abuse Mandatory Reporter Training is required within 6 months of hire and every 5 years thereafter. Basic Life Support Certification (BLS) will be required within 30 days of hire and every 2 years thereafter.
