Description:

Trinity Health Of New England is looking for an experienced Diabetes Educator - Registered Nurse RN to join our team.. As part of the diabetes educator team at the Diabetes and Endocrinology Center, this RN will work collaboratively with other educators and endocrinologists to provide the highest level of patient care and education training in both 1:1 and group settings.

The ideal candidate will teach patients both 1:1 and in group format how to manage their diabetes. This nurse should possess the Certified Diabetes Care and Education Specialist certification or should intend to obtain it within 2 years of hire, and should have experience with diabetes-related technologies including glucose meters, insulin pumps, and continuous glucose monitors.

You Will Be Responsible For:

  • Exceptional Patient Care: Assist Physicians with managing insulin dose adjustments for patients with all types of diabetes and will assist with managing a phone triage line for patients with diabetes-related questions. RN may be asked to administer endocrine therapies not specific to diabetes, for example, osteoporosis medications, under the direction of the endocrinologists.
  • Strong Educator: Teach patients how to use their personal and professional continuous glucose monitors.
  • Active Teamwork: Demonstrates problem solving, conflict resolution, and positive communication through teamwork. Will work collaboratively in a team environment but also enjoy some autonomy.

Requirements:

  • Education: Bachelors of Science in Nursing (BSN) degree
  • Experience: 2 or more years of clinical experience
  • Licensure: Current Licensure as a registered nurse (RN) in Connecticut
  • Certification: Certified Diabetes Care and Education Specialist (CDCES) certificate or work towards obtaining it within 2 years of hire.

Preferred Skills:

  • Experience: Diabetes Educator; Ambulatory services/chronic disease management
  • Bilingual: English/Spanish
