What you will do:

The Community Hospice has a full time opportunity for a social worker to join our hospice care team caring for patients in the Capital Region area as part of our Weekend Interdisciplinary Team caring for patients and families receiving hospice care. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position receives full time benefits, working 24 hours a weekend (8 hours - 8am to 4pm) Friday, Saturday, Sunday.

Responsibilities:

• Diagnose and treat psychological, behavioral and emotional disorders
• Provide specialized skills and mental health therapy for clients.
• Identify barriers which impact optimal patient functioning
• Ensure patients receive appropriate resources in most appropriate setting

What you will need:

• NYS SW Licensure
• Valid NYS Drivers license
• Minimum 2-3 years experience ideally caring for seniors
