Job Description Summary:
This position is responsible for providing supportive services to patients participating in a Behavioral Health Integration program within primary care offices. This role will help place phone calls to patients to discuss their wellbeing and coping skills, administer Depression and Anxiety screeners, complete billing documentation, provide resources for consistent counseling and psychiatry and verify insurance benefit coverage.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. This position requires ability to act quickly, good organization and time management and ability to work in high stress situations.
Minimum Qualifications:
Master's Degree: Social Work (Required)DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles, LSW - Licensed Social Worker - Social Work Certification and Licensure Board
Masters degree in Social Work/Counseling or a related field and LSW/LPC license in the State of Ohio.
1-2 years’ experience as an LSW/LPC, preferably in a patient-centric setting and knowledge of medical terminology and computer skills.
