Care Coordinator - Aftercare Specialist

Make a meaningful impact by helping patients transition safely and confidently into the next stage of their care.

As a Care Coordinator (Aftercare Specialist), you play a vital role in supporting patients as they prepare for discharge. You’ll provide thoughtful case management, coordinate services, and ensure each individual leaves with a safe, well‑planned path forward. This is a role for someone who thrives in a fast‑paced environment, communicates with clarity and compassion, and is committed to treating every patient with dignity and respect.

What You’ll Do

  • Develop safe, individualized discharge plans for patients across all age groups
  • Coordinate aftercare services and community resources
  • Maintain accurate documentation within the Electronic Health Record
  • Communicate effectively with patients, families, and interdisciplinary teams
  • Work independently to complete assignments while staying organized and assertive
  • Support a smooth transition of care by ensuring all follow‑up needs are addressed

What You Bring

Education

  • High School Diploma required
  • Bachelor’s degree in a human services–related field preferred

Experience

  • Two years of experience in mental health settings

Knowledge, Skills & Abilities

  • Basic to intermediate computer skills
  • Strong organizational skills and the confidence to advocate for patient needs
  • Ability to work with individuals of all ages
  • Solid understanding of Electronic Health Records
  • Excellent communication skills—clear, respectful, and patient‑centered
  • Ability to thrive in a fast‑paced environment with shifting priorities
  • Comfort working independently and following through on tasks to completion
