Stamford Hospital’s prestigious pharmacy team is seeking a skilled full-time Pharmacist to join the mission and ensure effective delivery of pharmaceutical care to all our patients. The Pharmacist will report to the Director of Pharmacy and is responsible for providing quality pharmaceutical care with an emphasis on accuracy and patient safety. The right incumbent for this role will demonstrate the knowledge and skills necessary to provide comprehensive, person-centered pharmaceutical services. These services include medication delivery, therapeutic recommendations, and promotion of optimal drug therapy through policy development.

Applicants must demonstrate flexibility and work rotating shifts as assigned. Primarily daytime and evening, with occasional overnight hours based on coverage needs including rotating weekends and holidays.
Responsibilities:
- Accurately verifies, prepares and dispenses medication orders per physician request according to established policies/procedures/protocols and based on pharmaceutical knowledge
- Provides timely and accurate drug information to healthcare professionals and lay persons when required
- Participates in medication counseling sessions for patients by reviewing side effects, indications and directions for use
- Demonstrates knowledge and skills necessary to provide quality pharmaceutical care to all patients.
- Ensures appropriateness of therapy and dosage for all patient populations including age-specific care
- Monitors patient therapy in accordance with departmental policies/procedures, protocols, and programs.
- Carefully identifies, prevents, intervenes, and follows up on any medication related problems, including any actual or potential adverse drug events
- Responsible for day-to-day management of drug distribution, including all controlled substances, IV compounded sterile medications, extemporaneously compounded medications, repackaged medications and investigational drugs
- Supervises pharmacy support personnel to ensure the seamless delivery of pharmacy services throughout the institution
- Completes required continuous training and education requirements, including state regulated, hospital and departmental specific requirement
- Ensures safe and appropriate cost-effective medication therapies for patients according to established policies, procedures and protocols.
- Assists with pharmacokinetic consult services and drug dosing per organizational protocol
- Ensures that therapeutic substitution policies are followed based on individual patient need.
- Participates in quality improvement and medication use review activities of the department
- Participates in hospital initiatives and committees as assigned
- Adheres to the hospital’s 340B requirements regarding ordering, dispensing and transfer of medications within the hospital departments and to outside hospitals
- Demonstrates fiscal responsibility
- Abides by regulatory requirements, including but not limited to USP797, USP800, USP795, NIOSH and Joint Commission standards
- Maintains current knowledge of drug use through continuing education
- Precepts pharmacy students as assigned
- Provides drug information in-services as assigned
Qualifications/Requirements:
- B.S. Pharmacy, Pharm.D. preferred.
- License or eligible for license as a Registered Pharmacist in Connecticut.
- Previous experience in a hospital setting preferred.
