Summary

The Health Services Coordinator (HSC) plays a crucial role at the Center of Occupational Health and Education (COHE) by monitoring claim caseloads to identify injured workers at risk of long-term disability and who coordinates care for those workers and facilitates communication between the worker, provider, employer, union (when applicable), and claims manager.

The HSC will help support claim coverage by monitoring claims, coordinating care, and facilitating communication. They manage a caseload of claims from an assigned territory of providers as determined by the COHE program manager and program director. Claim coordination activities include ensuring forms are received and complete; contacting injured workers, employers, providers, L and I staff, and other stakeholders to assist in the return-to-work process; and identifying return-to-work barriers and resources for resolution. In addition, the Health Services Coordinator will provide community outreach and organizing of provider volume and credentialing. This work can be done independent of the Program Manager and can bill at 100% of rate versus 90% that can be billed by an HSCA.

This position reports to the COHE program manager, with the opportunity for hybrid work (onsite and home) once fully trained as determined by the COHE program manager and director.

Essential Functions

  1. Coordinate and process health care provider applications and related verifications required for their enrollment in the COHE program, including clinic and facility leadership.
  2. Communicate to keep health care providers and facility staff informed of their application status and reaches out to them to gather all required information. Acts proactively to get new health care providers processed and to record any changes in personnel or contact information at participating health care organizations.
  3. Acts as resource to assist in referring patients to appropriate providers throughout the region.
  4. Health Service Coordinator billable activities include: Initial and follow up contact to physician, employer and/or injured worker once claim initiated for clarification of return-to-work program, light duty availability, transitional work plans, and to provide resource to inquiries about COHE. Billed activities will be reimbursed at 100% of the value of claims work.
  5. Establish relationships with health care providers participating in the COHE.
  6. Delivers professional presentations for appropriate groups promoting participation and/or support for the COHE.
  7. Triage and respond to questions from all customers. Documents necessary information in EMR and communicates to claim, regardless of billable status.
  8. Promotes a positive, calm, and effective work environment throughout the unit that contributes to staff recruitment and retention. Provides input for the performance appraisals of other staff members. Orients staff members when requested. Demonstrates an awareness of self-responsibility and accountability for the job at hand by utilizing worked hours efficiently.
  9. Attends all in-service education programs mandated by Confluence Health, state, and federal regulations. Conducts or assists with at least one in-service per year. Pursues educational activities to maintain a current level of knowledge and competency.
  10. Promotes a positive, calm, and effective work environment throughout the unit that contributes to staff recruitment and retention. Completes performance appraisals of staff members reporting to them. Orients staff members when requested. Demonstrates an awareness of self-responsibility and accountability for the job at hand by utilizing worked hours efficiently.
  11. Demonstrates commitment to continuous quality improvement in all job responsibilities. Participates in performance improvement activities. Contributes and participates in program development. Facilitates accomplishment of departmental goals.
  12. Performs other duties as assigned.

Qualifications

Required:

  • Bachelor's Degree or three years’ experience in direct patient care, return-to-work coordination, occupational health care, or equivalent.
  • Direct patient care experience must primarily include collaborating with attending providers to develop, implement, or maintain treatment plans. Direct patient care experience should include experience building and implementing plans to assist patients in:
    • Setting and monitoring goals
    • Monitoring ongoing recovery, and
    • Identifying and addressing barriers and mitigations.
    • Examples may include: nurse, medical assistant, vocational rehabilitation
    • counselors, or claim manager.
    • Direct patient care does not include referral coordinators, schedulers, clinical administrative staff, or ancillary services (examples: diagnostic care).
  • Able to obtain and maintain an L&I Provider ID, and bill HSC services. No reported issues with Labor and Industries.
  • Oral and written communication with health care professionals, patients, employers, and insurance staff and resources.
  • Data management and tracking.
  • Completing care coordination or similar documentation.
  • Analyzing and communication provider, care coordinator, and best practice reporting data.
  • Computer skills with proficiency in Word, Excel, and PowerPoint.
  • Must successfully complete L&I required training and/or testing within 90 to 180 days.
  • Must have valid driver’s license and acceptable driving record as this position will require the use of personal or rental vehicle. Expect occasional travel of up to 300 miles (round trip) or less; and occasional overnight stay(s).

Desired:

  • HSCA or HSC experience.
  • Experience or certification as a vocational counselor, nurse case manager, accredited case manager working with injured workers in an industrial insurance system or health care setting, or equivalent certification or experience in a health care setting such as a physical therapist or occupational therapist.
  • Bilingual English/Spanish with ability to pass a bilingual fluency/skills test with a chosen CH vendor.
