Primary Duties and Responsibilities

The mission of Hoag Memorial Hospital Presbyterian, as a nonprofit, faith-based hospital, is to provide the highest quality health care services to the communities it serves. The position of Program Manager of Faith Community Nursing Outreach is to support this mission by leading a comprehensive community-facing program supporting the paid and unpaid professional RNs serving whole person needs: mental, physical and spiritual needs of people within diverse faith communities and settings throughout Orange Co and the region.

  • Lead Hoag’s Faith Community Nursing Outreach Program providing a bridge between the hospital and the community through the paid and unpaid work of the Faith Community Nurses.
  • Supports the process of developing a challenging and realistic programmatic vision and goals that reflect Hoag & the Spiritual Care Department’s vision, mission, values and strategic objectives.
  • Assumes responsibility for the overall operations of the Faith Community Nursing Outreach Program including staff management and coordination of unpaid professional Faith Community Nurses
  • Designs and supports effective systems to demonstrate outcomes, comply with regulatory agencies’ requirements and represent Hoag’s values to the community.
  • Oversee annual community-based vaccine campaigns. Develop and manage Hoag and community-based flu vaccine clinics, including annual policy & procedure updates, staff training, vaccine administration, clinic evaluation and outcomes.
  • Maintain a strong public relations presence within the faith based and health/social services community.
  • Collaborate with Hoag Departments and community agencies to improve access to available clinical, spiritual, mental health and other social services.

Qualifications

Education and Experience Required:

  • Minimum 5 years’ experience as a registered nurse, including supervisory and program development experience
  • Master’s level education in nursing or other health related field
  • Must have successfully completed a Foundations of Faith Community Nurse Course or do so within 1 year of hire

Preferred:

  • 3 years as a Faith Community Nurse; PHN or other Public Health experience

License Required:

  • RN currently licensed as a registered nurse in good standing in the state of California
