Working under general supervision, the Physiatry Program Coordinator works in collaboration with a multidisciplinary team of physicians, advance practice providers, registered nurses, therapists, support staff and schedulers to develop and monitor performance activities and improvement initiatives for the Physiatry Program. The Program Coordinator plays a critical support role with shared accountability to both patients on the inpatient service and the ambulatory care continuum to systematically plan and coordinate directed care. The Program Coordinator tracks diverse and medically complex Physiatry patients through prescribed therapeutic services for patients and families providing support to improve access to care, patient care, and outcomes. The Program Coordinator serves as a resource and primary contact to assist the patient and family in navigating prescribed therapies.
The Physiatry Program Coordinator manages the planning and implementation of the Adaptive Sports Program as well as other events, including educational conferences, annual picnics, family social engagements and other patient related recognition.
The Physiatry Program Coordinator is accountable for clerical support. Collects, compiles and analyzes moderately complex data and information; composes straightforward written descriptions of results. Requires the ability to exercise independent judgment and employ basic reasoning skills. The incumbent will use various PC software packages such as spreadsheets, word processing, graphics, databases, etc., to produce quality reports, presentations, or other documents.
