POSITION SUMMARY

To be fully engaged in providing Quality/No Harm, Customer Experiences, and Stewardship by

playing an integral role in the coordination and conduct of day-to-day valve program activities

under the direction of the Director of Cardiology and in collaboration with the valve program

team. Must be able to establish priorities and a course of action for managing several activities at

once while observing deadlines, and to function and communicate effectively with

multidisciplinary team members. The Valve Clinic Coordinator will participate in creating an

environment that implements the mission, vision and values of the organization.

PRIMARY ACCOUNTABILITIES

QUALITY/NO HARM:

1. Coordination of patient evaluation and screening processes

2. Contact with referring providers

3. Review clinical progress of patients and coordinates communications to referring, valve

and primary care physicians along with the research team.

4. Plan, organize and provide direct care to the valve patient.

5. Receipt and review of pertinent clinical records

6. Facilitate the development of a patient specific teaching plan

7. Coordination of medical record review with other team members

8. Oversight of valve clinic schedule and processes

9. Involvement in patient and family education and consent

10.Patient tracking and triage

11.Coordination of inter-departmental processes and communication

12.Coordination of valve program team meetings/communication processes

13.Education of staff members as indicated

CUSTOMER EXPERIENCE:

1. Maintenance of patient confidentiality in accordance with HIPAA regulations

2. Assistance in training of new valve program team members as appropriate

3. Encourages collegiality when integrating valve services with other departments

STEWARDSHIP:

1. Reimbursement contact for patient's and family members

2. Other related responsibilities as indicated

Work Experience

MINIMUM QUALIFICATIONS

  • Education: Graduate of an approved school of professional nursing.
  • Licensure: Current Florida RN licensure or endorsement.
  • Certification: Must hold current AHA BLS Healthcare Provider Completion Card prior to

start date and maintain.

PHYSICAL REQUIREMENTS

  • Must have visual acuity and hand-eye coordination to perform clinical tasks.
  • Must have corrected vision and hearing to normal range.
  • Must be able to sit and stand for extended periods of time.
  • Potential for exposure to bio-hazardous waste and known or unknown diseases.

MENTAL DEMANDS

▪ Must have ability to work in often stressful environment with patients/families at various ages and

levels of understanding.

▪ Must have ability to concentrate on multiple priority activities.

▪ Must have ability to respond appropriately to life threatening situations.

▪ Must have critical thinking and problem solving skills
