Summary of Duties
As a Human Services Case Manager, you will:
- Interview Oregonians to determine initial and ongoing physical, cognitive, and financial eligibility for healthcare, social services, and protective service needs.
- Conduct interviews in person or on site using the Case Assessment and Planning System (CA/PS) assessment tool on a state-issued laptop.
- Make appropriate referrals to other ODHS programs and resources as well as community resources.
- Process data according to policy timeframes, ensure appropriate documentation and forms are completed and sent, and communicate eligibility decisions to ensure delivery of benefits and services.
- Contact Oregonians or collateral contacts monthly to monitor service plan and safety needs.
- Work closely and collaboratively with the Oregonian, other agencies, medical providers, and community partners in order to provide benefits, services, and information.
- Work with Oregonians to create person-centered risk mitigation plans and provide risk mitigation as needed.
Minimum Qualifications
A valid driver’s license and acceptable driving record are required for this position.
A bachelor's degree in a Behavioral Science, Social Science, or a closely related field.
OR
A bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
OR
An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
OR
Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

Preference will be given to candidates that are bilingual in Russian and Vietnamese.

Working Conditions
- Your work schedule will be 8 a.m. - 5 p.m., Monday – Friday Pacific Standard Time (PST).
- Hybrid work condition will be considered after 6-month trial service.
- Travel is required to meet business needs and to attend employee training classes. Reasonable accommodations per the ADA or creative solutions in certain cases may be used to meet this need.
- This position requires a good attendance pattern and appropriate use of accrued paid leave to ensure branch coverage for vacations and other leaves, as well as acceptable customer service for the Oregonians we serve.
- You will interact with a wide range of people including some who have experienced trauma and may exhibit challenges in controlling their emotions.
- Employees in these positions may be required to work a flexible work schedule as a condition of employment.
- Fast paced environment with constantly evolving policies and procedures.
- May drive a variety of different types of vehicles to remote places in a variety of terrain and possibly inclement weather.
- Repetitive motions, prolonged sitting, and daily computer and technology use.
- Work is conducted both in office settings, long term care facilities, in clients’ homes, and the community at large.
- May come in contact with animals.
- May be exposed to communicable diseases, such as the flu virus, Covid-19, and MRSA.
