Assist in the operation of the population health department and is driven to meet and exceed customer expectations. Responsible for achieving timely patient contact and documentation thereby influencing quality metrics. Holds self and team accountable for achieving the highest level of quality of care, customer service, and employee satisfaction, and recognizes the contribution for desired results.
JOB SPECIFICATIONS AND CORE COMPETENCIES
Education: Associate must be a high school graduate or have successfully completed a General Education Development program.
Licensure: Current Licensure as a Pharmacy Technician by the State of Indiana. Certified Pharmacy Technician (CPhT) preferred. Valid drivers license.
Experience: Six months of hospital/institutional pharmacy or community or ambulatory pharmacy experience. Six months of inpatient, community, or ambulatory pharmacy clerical experience preferred. Experience in Population Health,
Managed Care-Quality Programs preferred
