Summary:
LNA I
Under the direct supervision of licensed nursing staff, provides essential support and direct care to residents by assisting with daily living activities, monitoring health conditions, and ensuring a safe, clean, comfortable, and enriching environment.
LNA II
With direction and oversight by licensed nursing staff, independently provides essential support and direct care to residents by assisting with daily living activities, monitoring health conditions, and ensuring a safe, clean, comfortable, and enriching environment. Mentors new nursing assistants.
LNA III
With general oversight by licensed nursing staff, independently provides essential support and direct care to residents by assisting with daily living activities, monitoring health conditions, and ensuring a safe, clean, comfortable, and enriching environment. Mentors/instructs new nursing assistants and makes suggestions for care, treatment, and practice/procedural improvements.

Each additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education.
Minimum Qualifications:
LNA I
Education/Experience:
High school diploma or equivalent credential
License/Certification:
- NH Nursing Assistant license
- Driver’s license
Other Requirements:
- May be required to work non-traditional work hours to meet the needs of the facility
- Must possess physical stamina for prolonged standing and to perform tasks such as lifting, transferring, and transporting residents and materials
PREFERRED QUALIFICATIONS:
Experience in long-term or geriatric care setting or other role involved in the provision of medical care. Strong verbal and written communication and interpersonal skills. CPR and First Aid certification.
31113001BGA01 (Level 1 to Level 2)
For promotion from the 31-1130 Nursing Assistants, Orderlies, and Psychiatric Aides-1 job title to the 31-1130 Nursing Assistants, Orderlies, and Psychiatric Aides-2 job title as a Residential LNA, the eligibility requirements in Personnel Rule 303.08Broad Group Level Advancement must be satisfied, as well as the following criteria:
- Has one year of experience functioning as a Licensed Nursing Assistant or other practicing medical professional
- Demonstrates sufficient competency in performance of required tasks to serve as mentor for new nursing assistants
- Receives a satisfactory performance evaluation within the past 12 months
- Is up-to-date on all mandatory in-house training
- Is recommended for promotion by supervisor(s) and approved by the director of nursing
LNA II
Education/Experience:
Equivalent combination of 1 year education and experience after completion of high school
License/Certification:
- NH Nursing Assistant license
- Driver’s license
Other Requirements:
- May be required to work non-traditional work hours to meet the needs of the facility

For the in-band advancement to be applied, the eligibility requirements in Personnel Rule 303.09 In-Band Advancement must be satisfied, as well as the following criteria:
- Holds a valid NH Medication Nursing Assistant license
- Assumes responsibility for administering medications
31113002BGA01 (Level 2 to Level 3)
For promotion from the 31-1130 Nursing Assistants, Orderlies, and Psychiatric Aides-2 job title to the 31-1130 Nursing Assistants, Orderlies, and Psychiatric Aides-3 job title as a Residential LNA, the eligibility requirements in Personnel Rule 303.08 Broad Group Level Advancement must be satisfied, as well as the following criteria:
- Holds five NH Nursing Board Approved competencies
- Receives a satisfactory performance evaluation within the past 12 months
- Is up-to-date on all mandatory in-house training
- Is recommended for promotion by supervisor(s) and approved by the director of nursing
LNA III
Education/Experience:
Equivalent combination of 3 years of education and experience after completion of high school
License/Certification:
- NH Nursing Assistant license
- Driver’s license
Other Requirements:
- Satisfactory completion of five NH Nursing Board Approved competencies
- May be required to work non-traditional work hours to meet the needs of the facility
- Must possess physical stamina for prolonged standing and to perform tasks such as lifting, transferring, and transporting residents and materials
PREFERRED QUALIFICATIONS:
Strong preference for candidates with at least three years of experience as a Licensed Nursing Assistant in a long-term or geriatric care setting. Strong verbal and written communication and interpersonal skills. CPR and First Aid certification.

Responsibilities:
LNA I
- Assists residents with activities of daily living (ADLs), including getting out of bed, bathing, dressing, grooming, toileting, standing, walking, or exercising. Ensures proper dietary intake and adequate hydration and that dietary restrictions and preferences are observed. Supports mobility by helping residents move between beds and wheelchairs, and transferring residents to special functions, trips, appointments, and other activities within and outside the facility.
- Utilizes appropriate equipment and follows established safety protocols to ensure resident’s comfort and dignity. Adapts techniques and frequency based on individual needs and preferences and established care plans. Promotes independence in meeting individual needs.
- Acts in accordance with established care plans; participates in Plan of Care meetings as assigned/needed. Documents care provided and individual’s behavior/response; maintains accurate records. Monitors and records vital signs such as temperature, blood pressure, and pulse, as well as food/liquid intake and urinary/fecal output. Collects specimens for routine laboratory testing. Reports changes in residents’ physical, mental, or emotional condition to a supervisor or more senior nursing staff in a timely manner.
- Replenishes charts and retrieves, stocks, and distributes medical supplies and personal protective equipment (PPE) as required. Reports any shortages.
- Follows infection control protocols and safety procedures. Maintains cleanliness and hygiene in resident rooms and common areas. Performs daily assigned tasks such as clothing distribution, emptying laundry hampers, and cleaning furniture.
- Prepares residents for appointments and medical team consults. Provides a level of security/support for the resident.
- Demonstrates empathy and patience in providing care and assistance. Encourages and maintains open communication and positive interactions with residents, families, guardians, and other visitors.
- Attends and participates in in-service educational programs as well as organization meetings and committees. Responds to supervision and works cooperatively with others in provision of care and operation of facility.
LNA II
- Exercises some independent judgement in providing daily care and resolving minor physical and emotional needs. Assists residents with activities of daily living (ADLs), including getting out of bed, bathing, dressing, grooming, toileting, standing, walking, or exercising. Ensures proper dietary intake and adequate hydration and that dietary restrictions and preferences are observed. Supports mobility by helping residents move between beds and wheelchairs, and transferring residents to special functions, trips, appointments, and other activities within and outside the facility. Communicates resident needs to others when outside own scope of duties.
- Utilizes appropriate equipment and follows established safety protocols to ensure resident comfort and dignity. Adapts techniques and frequency based on individual needs and preferences and established care plans. Promotes independence in meeting individual needs, while recognizing variations in capabilities and limitations.
- Performs treatment procedures within the LNA scope of practice. Follows medical orders and acts in accordance with established care plans. Participates in Plan of Care meetings as assigned/needed; presents patient/resident information when applicable. Coordinates visits from consulting professional health team members, including dentists, podiatrists, and physical therapists. Assists health team members and provides a level of security/support for the resident.
- Documents care provided and individual’s behavior/response; maintains accurate records. Monitors and records vital signs such as temperature, blood pressure, and pulse, as well as food/liquid intake and urinary/fecal output. Assists with screening for medical issues and collects specimens for routine laboratory testing. Observes and reports changes in residents’ physical, mental, or emotional condition to a supervisor or more senior nursing staff in a timely manner.
- May serve as mentor for new nursing assistants providing guidance on established facility/unit responsibilities, policy and procedures to meet the standards of care.

- Attends and participates in in-service educational programs as well as organization meetings and committees. Responds to supervision and works cooperatively with others in provision of care and operation of facility.
LNA III
- Mentors and instructs new nursing assistants on facility/unit responsibilities and established policies and procedures to meet the standards of care.
- Exercises independent judgement to determine and prioritize resident needs. Assists with activities of daily living (ADLs), including getting out of bed, bathing, dressing, grooming, toileting, standing, walking, or exercising. Recommends supplemental assistance to meet ADLs. Ensures proper dietary intake and adequate hydration, and that dietary restrictions and preferences are observed. Supports mobility by helping residents move between beds and wheelchairs, and transferring individuals to special functions, trips, appointments, and other activities within and outside the facility. Communicates resident needs to others when outside own scope of duties.
- Utilizes appropriate equipment and follows established safety protocols to ensure resident comfort and dignity. Adapts techniques and frequency based on individual needs and preferences and established care plans. Promotes independence in meeting individual needs and demonstrates a keen understanding of each individual’s capabilities and limitations.
- Performs treatment procedures within the LNA scope of practice. Collaborates with the healthcare team to plan and implement treatment plans to address complex patient/resident needs. Follows medical orders and acts in accordance with established care plans. Makes recommendations for treatments/therapies and changes in care and participates in Plan of Care meetings as needed. Coordinates visits from consulting professional health team members, including dentists, podiatrists, and physical therapists. Assists health team members and provides a level of security/support for the resident.
- Documents care provided and individual’s behavior/response; maintains accurate records. Monitors and records vital signs such as temperature, blood pressure, and pulse, as well as food/liquid intake and urinary/fecal output. Assists with screening for medical issues and collects specimens for routine laboratory testing. - Manages inventory, prepares/submits supply orders, and prepares work orders for equipment repairs and maintenance needs. Ensures charts, medical supplies, and personal protective equipment (PPE) are stocked and distributed as required.
- Follows infection control protocols and safety procedures and recommends practice changes when needed. Maintains cleanliness and hygiene in resident rooms and common areas. Performs daily assigned tasks such as clothing distribution, emptying laundry hampers, and cleaning furniture.
- Demonstrates empathy and patience in providing care and assistance. Encourages and maintains open communication and positive interactions with residents, families, guardians, and other visitors. Refers questions to appropriate senior staff.
- Assists with basic social problems and facilitates the adjustment of residents. Guides residents in therapeutic activities to enhance residents’ quality of life and maintain ADL skills.
- Attends and participates in in-service educational programs as well as organization meetings and committees. Responds to supervision and works cooperatively with others in provision of care and operation of facility.
