Qualifications:
- Current, unencumbered Nurse Practitioner (NP) license in the state of Illinois.
- Minimum of 2 years of clinical experience; hospice or palliative care experience a plus.
- Strong assessment, communication, and documentation skills.
- Ability to work independently, manage time effectively, and adapt to a PRN schedule.
- Valid driver’s license, auto insurance, and reliable transportation (travel required to patient homes and facilities).
HOURS: PRN As Needed Monday-Sunday 8:30AM-5PM for Face to Faces for Hospice Patients
TERRITORY: Crystal Lake, Johnsburg, McHenry, IL.

What You’ll Do:
- Conduct Medicare-required Face-to-Face (F2F) encounters to determine ongoing hospice eligibility.
- Perform patient assessments focused on clinical status, disease progression, and functional decline.
- Document findings in accordance with CMS regulations and agency policies.
- Collaborate with interdisciplinary team members, including physicians, nurses, social workers, and chaplains, to support care planning.
- Communicate assessment outcomes clearly and effectively with both patients/families and care teams.
- Provide clinical insight and education regarding disease trajectory and hospice appropriateness.
- Maintain timely and accurate completion of all required documentation in the electronic health record.
