Key Responsibilities
- Provide timely care for patients presenting with acute, non-life-threatening conditions in a same-day clinic setting.
- Acute needs:
- Examples: URI, bronchitis, otitis, rash, UTI, sprains/strains
- Urgent needs:
- Examples: acute abdominal pain w/o severe symptoms, injuries, migraines, nausea/vomiting, blood pressure elevation
- Med Refill visit if visit with Care Team is not available in timely fashion.
- Sports physicals
- Annual Wellness Visits if needed based on calendar constraints.
- Collaborate with clinical teams to triage patients and determine the urgency and type of care required.
- Collaborate with advanced practice providers, nurses, and other healthcare team members to ensure continuity and quality of care.
- Refer patients to specialty care, primary care follow-ups, or emergency services as appropriate.
- Close care gaps (tee up labs for open lab gaps and help reschedule patient with PCP)
- Close HCC gaps
- Help set up follow up appointments with PCP for needed care.
- Work closely with the care team and scheduling to help direct patients to the most appropriate level of care.
Additional expectations:
- Conduct thorough assessments, including history taking, physical exams, and diagnostic testing as necessary.
- Collaborate with physicians, specialists, and other healthcare professionals to ensure continuity of care.
- Perform minor procedures such as suturing, wound care, incision and drainage, and splinting.
- Manage patient flow efficiently, ensuring same-day access and reducing wait times.
- Utilize electronic health records (EHR) to document patient encounters accurately and in a timely manner.
- Maintain up-to-date knowledge of clinical guidelines and best practices in urgent care and primary care settings.
- Participate in quality improvement initiatives and adhere to clinic protocols, policies, and procedures.
- Provide compassionate, culturally sensitive care to patients of all ages, ensuring a positive patient experience.
Qualifications
- For Physicians (MD/DO): Board-certified/eligible in Family Medicine. Valid state Missouri and Kansas medical license.
- For Advanced Practice Providers (NP/PA):
- NP: Certified Nurse Practitioner with a focus in Family Medicine or Acute Care. Current state Missouri and Kansas licensure and certification.
- PA: Certified Physician Assistant with experience in primary care, urgent care, or emergency care settings. Current state Missouri and Kansas licensure and certification.
- Excellent communication, interpersonal, and teamwork skills.
- Ability to work in a fast-paced environment and manage multiple patients simultaneously.
- Proficiency in the use of EHR systems.
