- Navigate patient care needs that involve evaluation and interpretation, as the primary point of contact for PrEP/PEP patients and program partners, escalating complex issues to the Manager, Clinical Operations & PrEP/PEP or the PCIII PrEP/PEP/LAI Coordinator Sr.
- Using knowledge of the Ryan White Program and industry best practices, demonstrate creativity and innovation in case management to address and overcome barriers in the healthcare system.
- Conduct initial and ongoing in-clinic assessments as required by clinical guidelines and federal and state funders, and monitor incoming referrals from various sources.
- With direction from the Operations Manager and the PCIII PrEP/PEP/LAI Coordinator Sr. facilitate an individualized comprehensive care plan that integrates life-saving medical and social services, as well as treatment adherence support, which includes assessing patient financial support needs, identifying and securing funding and resource acquisition, navigating the complicated healthcare system, and addressing complex insurance issues.
- Under moderate guidance, manage a caseload that requires organizational skills with the ability to multi-task, and track patient needs from intake through discharge from the program.
- Maintain accurate and compliant documentation of activities in designated databases, adhering to ethical guidelines, state regulations, and internal policies.
- Maintain the integrity, usability, and confidentiality of program data and databases, to support seamless patient care, and the accurate capture of referrals and outcome dispositions.
- Serves as patient advocate working with insurance carriers, medical providers, social services, etc., that facilitate patient needs and ensure funded service objectives and goals are met.
- Maintain effective communication with the multidisciplinary team and external partners to enhance patient outcomes and ensure seamless care coordination.
- Educate and council patients on risk reduction and healthy behaviors amongst other topics that facilitate needs being met and quality of life.
