Job Summary

The Complex Care Program Manager is responsible for assisting Mass General Brigham in executing the CCM strategic vision related to post-acute capacity management and the complex care strategy across the continuum. Care continuum management is a multi-disciplinary pursuit whose mission is to ensure that all patients receive high quality and safe care that is seamless and coordinated across different locations and occurs in the most appropriate care setting for a given patient and/or population.

The Complex Care Program Manager is responsible for managing all aspects of new Complex Care programs and corresponding projects. Implements and supports CCM initiatives that support alignment with organizational goals and strategic planning. Leads the communication and collaboration with stakeholders. Develops and supports program processes, analyzes data, prepares reports, and provides regular updates to program leadership. Identifies and escalates issues and risks and recommends solutions. Leads and provides guidance and support to other CCM team members.

Qualifications

  • Supports Mass General Brigham CCM strategy efforts related to capacity management and throughput to ensure patients transition at the right level of care; right time.
  • Leads the long-term care referral management process, triage process, escalations and post-acute relationship management.
  • Leads the complex care program planning efforts, including ongoing evaluation of system needs.
  • Responds to team referrals and escalations from members CCM, individual providers or others.
  • Utilizes Epic, 4-Next to identify and track patient care progression.
  • Communicates with CCM team members or multi-disciplinary team and / or other health care clinicians to or to alert providers and team about potential issues and solution to barriers.
  • Works in collaboration with other members of the CCM team and leadership to develop standards of care
  • Assists in training and educating regarding complex care, capacity management and transition planning
  • Networks with post-acute-care providers, discharge agencies, managed care networks and third-party payors.
  • Conducts research, manages and analyzes data in systems, and develops reports and presentations to support programs.
  • Identify potential risks, issues, and challenges associated with CCM programs and develop mitigation strategies to minimize impact.
  • Maintains excellent relationships with leaders of post-acute care organizations both within and external to Mass General Brigham; acts as a link between Mass General Brigham and these organizations.
  • Ensures quality metrics such as length of stay are evaluated on a routine basis
  • Along with CCM leadership, reviews clinical and financial outcomes and recommends then executes operational adjustments as indicated.
  • Performs other tasks as assigned by the Executive Director of Complex Care and Care Continuum.

Education

  • Bachelor's Degree Related Field of Study required
  • Master's Degree preferred

Licenses and Credentials

Experience

Case Management/Discharge Planning/Complex Care Experience, minimum of 3+ years

Academic Health Care Environment Experience, 3-5 Years

Program Management Experience 2-3 years required

Knowledge, Skills and Abilities

  • Strong care coordination experience with knowledge to differentiate levels of care and develop strategies to support patients and families.
  • Strong leadership and project management skills, with the ability to effectively coordinate and delegate tasks, manage timelines, and drive results.
  • Excellent interpersonal and communication skills, with the ability to engage and collaborate with medical professionals, faculty, and staff at all levels
  • Strong competency working with hospital computer systems and case management systems.
  • Competent using Microsoft Word, PowerPoint, and Excel.
  • Ability to interpret clinical and financial data related to program performance.
  • Demonstrated ability to present and speak in front of groups.
  • Ability to handle routine work, unexpected priorities, and multi-task.
