Position Overview:

Successful candidate will have Hospice experience.

Under the supervision of the Director of Patient Care Services (DPCS), the Admissions Nurse, in cooperation with Hospice Administrator and the Hospice Medical Director facilitates the admission of patients to hospice services in the patient's place of residence in alignment with SCAH, mission, goals and objectives. The Admissions Nurse is responsible for conducting an initial nursing assessment of a potential hospice patient and reporting the patient's condition to the Hospice Medical Director using the applicable Local Coverage Determinations. If the Hospice Medical Director supports the hospice diagnosis and gives verbal certification of terminal illness, the Admissions Nurse will admit the patient to hospice services and attend to the immediate needs of the patient. The Admissions Nurse will document the assessment, interventions and outcomes in the patient's chart utilizing the electronic medical record at the point of care.

EDUCATION

  • Equivalent experience will be accepted in lieu of the required degree or diploma.
  • Other: Graduate from an accredited school of nursing with a current California Registered Nursing license.
  • Other: California Public Health Certificate OR
  • Other: Completion of Community Health Nursing program.

CERTIFICATION & LICENSURE

  • RN-Registered Nurse of California
  • BLS-Basic Life Support Healthcare Provider
  • DL-Valid Drivers License Class C
  • AUTO-Automobile Insurance

PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:

  • 1 year of acute medical experience.
  • 5 years direct patient care experience.
  • Experience with admissions or re-certifications under Region IX Local Coverage Determinations (LCDs) strongly preferred.

SKILLS AND KNOWLEDGE

  • Possess critical thinking skills, in-depth knowledge of clinical disease entities common to hospice and palliative care for terminally ill patient.
  • Possess knowledge of the hospice philosophy, the Medicare hospice benefit, Conditions of Participation, and Local Coverage Determinations under the Medical Fiscal Intermediary (FI).
  • Should also possess knowledge of hospice reimbursement methods both under government benefits and commercial payors.
  • Must have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independent, sound nursing judgment in planning and providing patient care.
  • Ability to recognize the special needs of Hospice patients and others in the home.
  • Ability to communicate with patients and significant others in an effective, mature, caring manner.
  • Ability to maintain harmonious constructive relationships with internal and external customers.
  • Demonstrates clear effective written and verbal communication skills in English.
  • Be proficient in utilizing cellular phone technology including smart phones or PDAs and have strong computer skills.
  • Ability to be aware of surroundings and respond to emergency and/or urgent events.
  • Ability to select and utilize patient transfer/transport equipment safely.
  • Productivity standards – ability to meet productivity expectations.
  • Must be detail-oriented.
  • Ability to work in mentally and physically demanding environment

Pay Range is $67.22 to $90.33 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
