GENERAL SUMMARY:

The Program Manager, Value-Based Integration, serves as a key administrative and operational leader supporting value-based contracting initiatives across Henry Ford Health value-based contracting networks, including the Mosaic Clinically Integrated Network (CIN), Mosaic ACO, and the Henry Ford Physician Network (HFPN). This role is responsible for managing programs and executing projects that drive the strategic, clinical, and financial goals of the networks. The Program Manager coordinates cross-functional initiatives, interprets data to inform decisions, oversees project timelines, and ensures successful delivery of critical outcomes that advance the mission of value-based, patient-centered care.

The Program Manager also leads or contributes to complex projects that enhance operational systems and performance across Henry Ford Health. This includes responsibility for project planning, budgeting, resource allocation, reporting, and implementation. The role may serve as a consultant on enterprise-wide initiatives and provides oversight to project support staff. Additionally, the Program Manager develops and applies continuous improvement strategies to increase engagement and optimize performance throughout the network and affiliated organizations.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Partners with leaders across the Mosaic CIN, HFPN, and Mosaic ACO—including operations, contracting, finance, and care delivery—to advance value-based care and contracting efforts. Collaborates with key clinical and operational stakeholders to identify strategic opportunities that enhance network alignment and integration.
  • Manages and oversees the execution of network engagement strategies through programs and projects that support value-based care contracting goals, including achievement of contractual metrics and overall contract success.
  • Maintains both a broad perspective and focused understanding of value-based care and contracting to inform forward-thinking strategies and recommendations.
  • Contributes to the design and implementation of systems and workflows that establish and sustain effective performance management processes.
  • Prepares executive-level presentations, dashboards, and reports to enhance understanding among senior leadership, boards, and committees.
  • Builds and maintains strong working relationships with operational, physician, and communications leaders to ensure accurate and timely messaging across the network and broader organization.
  • Pursues professional development and cross-training opportunities to deepen knowledge of value-based care and expand functional versatility within the organization.
  • Independently manages complex projects involving multiple departments and teams, ensuring alignment with operational improvement goals across Henry Ford Health.
  • Conducts project-based assessments of assigned areas; serves as project lead for individual or team-based initiatives, including responsibility for planning, scheduling, resource coordination, and issue resolution.
  • Develops and refines project scopes, objectives, deliverables, and timelines in collaboration with stakeholders.
  • Recommends resource allocation strategies and contributes to budget development and planning.
  • Leads and guides day-to-day activities of project teams; facilitates meetings and team collaboration to maintain progress and accountability.
  • Provides periodic updates to senior leadership through project reports and verbal presentations, summarizing progress, findings, and recommendations.
  • Translates strategic concepts into actionable project plans, establishes priorities, and monitors progress to ensure successful execution and goal attainment.
  • Documents current-state operations and systems, identifying opportunities for process and performance improvement.
  • Evaluates and tests alternative processes, identifies associated costs and benefits, and proposes data-informed solutions.
  • Defines requirements for procedural changes or system redesigns and supports implementation of new or revised approaches.
  • Guides support staff in completing operations analysis and decision support tasks; ensures alignment with project goals and timelines.
  • Establishes priorities and work schedules for assigned staff and projects to maintain controlled, efficient workflow.

EDUCATION/EXPERIENCE REQUIRED:

  • Bachelor’s Degree in a related field, preferably in Healthcare Administration, Public Health, or business function. Master’s degree preferred.
  • Five (5) years of experience in healthcare required, with at least three (3) years of experience in value-based contracting or healthcare administration.
  • Five (5) years of experience as a consultant, analyst, or strategist.
  • Proven ability to manage multiple priorities, analyze and interpret data, and develop effective project plans and executive summaries for various stakeholder audiences.
  • Self-directed, detail-oriented, and results-driven.
  • Demonstrates strong initiative, flexibility, and problem-solving skills.
  • High proficiency in project management software, Microsoft Office Suite (Excel, PowerPoint, Word), and virtual meeting platforms.
  • Commitment to teamwork, integrity, and continuous learning.
  • Ability to apply innovative solutions to problems and familiarity with TQM process is also required. Lean training highly preferred.
  • Knowledge of value-based contracting and familiarity with risk/reward models (e.g., MSSP, Direct-to-Employer, etc.).

CERTIFICATIONS/LICENSURES REQUIRED:

  • PMP or similar project management certification preferred but not required.
  • Must meet or exceed core customer service responsibilities, standards and behaviors as outlined in the Henry Ford Health Customer Service Policy
  • Must practice the customer skills as provided through on-going training and in-services.

PHYSICAL DEMANDS/WORKING CONDITIONS:

Normal office environment with minimal exposure to noise, dust, or extreme temperatures.
