This includes in detail but is not limited to: AccessHealth Lakelands, SRH/ECH Healthy Outcomes Plan, SBIRT programing, Prevention and Wellness services, Community Health Services Mobile Clinic, Community Health Needs Assessment (CHNA), Community Benefit Reporting (CBR), CMS Health Equity Framework and Measures, and all community outreach services for education and coordination, etc. This includes identifying opportunities and needs as well as guiding
teams in strategic planning, project management, and financial management of implementation and operation of those services.
