Advanced Nurse Practitioner/Heart Transplant Program Coordinator, Morristown

The Heart Transplant Program Coordinator oversees the multidisciplinary team in all aspects of ventricular assist devices in both inpatient and outpatient clinical settings and can assume clinical duties. The VAD Program Coordinator works directly with the Medical Director, the Surgical Director, and the Director of Interventional Cardiology to oversee and drive processes to improve patient outcomes. The VAD Program coordinator promotes collegiality within the multiple departments caring for the VAD patients including education, process improvement, delivery of care and data abstraction.

Position Responsibilities:

  • Assist with VAD initiatives – including but not limited to, reduction in length of stay, observed to expected mortality, 30-day readmissions, adherence to core measures and other publicly reported data, optimized transitions of care.
  • Work with administrative director to oversee inpatient and outpatient VAD programs
  • Represent the VAD program and its activities within the AHS system and hospital level committees as needed
  • In collaboration work with the surgical and medical directors of advanced heart failure, oversight of advanced heart failure mechanical circulatory support program
  • Work collaboratively with a variety of professional, administrative and community leaders to raise VAD awareness.
  • Improve operational efficiency of VAD management including maintenance of computer-based reporting and data

Essential Skills and Experience:

  • Licensed Advanced Practice Provider from an accredited college or university
  • Ability to communicate clearly and in a supportive manner with patients and/or families regarding condition, prognosis, health needs and available services
  • Extensive knowledge of current trends and developments in the care of VAD patients
  • Ability to develop and maintain effective working relationships with professional staff, local officials, and the public.
  • Keeps abreast of current research and incorporates relevant findings into specific areas of clinical practice regarding the care of the VAD patient
