Assessment Coordinator

About Us

Woodland Springs is a 96-bed behavioral health hospital located in Conroe, Texas. We provide inpatient and outpatient programming for those facing mental health and addiction challenges. Our programs offer caring, compassionate treatment for children, adolescents, adults, and seniors.

Programs include inpatient mental health and addiction treatment, Partial Hospitalization Programs (PHP), Intensive Outpatient Programs (IOP), and Electroconvulsive Therapy (ECT).

Job Summary

The Assessment Coordinator is responsible for conducting pre-admission assessments and working with the interdisciplinary team to ensure appropriate patient placement and support.

Responsibilities

  • Conduct initial assessments for incoming patients to determine appropriate level of care.
  • Collaborate with physicians, clinicians, and intake staff during the admission process.
  • Provide accurate documentation and communicate findings clearly to care team.
  • Maintain knowledge of insurance requirements and pre-authorization procedures.
  • Support patients and families through the intake and transition process.

Qualifications

  • Master’s degree in Social Work, Counseling, Psychology, or related behavioral health field required.
  • Licensure (LMSW, LCSW, LPC, LMFT) preferred.
  • Previous experience in intake, assessment, or clinical coordination preferred.
  • Strong clinical judgment, communication, and documentation skills.
