JOB SUMMARY
Under general supervision, this position is responsible for maintaining the integrity of the content of the HHCS centralized electronic health record by performing in-depth investigation of questionable data entry and correcting as appropriate. The Chart Completion Analyst provides issue identification, assessment, resolution, and technical support in order to achieve desired outcomes and compliance with HHCS policies/procedures and JC standards and other regulatory agency standards.
This role analyzes chart correction scenarios and performs subsequent hands-on technical chart corrections as part of daily work responsibilities. This position serves as an organization resource for issues involving chart correction, identity, and instructional support. The Chart Completion Analyst works at the center of the chart correction process, facilitating correction of clinical information throughout all inpatient and outpatient information systems. The individual coordinates the parties and interdisciplinary departments involved in each case (i.e., Patient Access, clinical departments, Pharmacy, Radiology, Lab, Physician Offices, Clinics) and is responsible for ensuring all necessary corrections are made in a timely, and each case is validated prior to closure to ensure that documentation is complete and accurate to support patient safety and continuity of care.
The position performs quantitative and qualitative analysis of medical records of discharged Inpatient, Observation, and Outpatient Surgery records in accordance with HHCS policies/procedures and JC standards and other regulatory agency standards. The electronic medical record is reviewed for missing documents, incomplete information on existing documents, and missing signatures to ensure the record is complete and accurate. Electronic deficiencies are assigned to the proper physician to complete. Changes to the record that require reanalysis are also reviewed and additional action is taken. Assistance is provided to physicians when they are completing their deficiencies.
JOB QUALIFICATIONS
Education: High school graduate or GED equivalent required.
Licensure: RHIT certification preferred.
