Targeted Case Manager will help individuals develop skills, access resources, and learn to self-manage illnesses in order to be successful in life, work, education, and social settings. Targeted Case Managers teach, model, and practice skills with persons served in order to increase self-sufficiency and independence. They provide daily, direct, and indirect services to an assigned caseload; meeting with the person served where they are at in order to assist the individual with daily living skills, educating them regarding diagnosis and treatment, accessing services and resources as needed including financial assistance, medical assistance, housing assistance, crisis services, and meeting their immediate needs of food, clothing, and shelter. The Targeted Case Manager will outreach incoming referrals to CPRC services, engaging clients and assisting with resources until the client is assigned to a CSS.
