Overview:

0.8 FTE, 32 hours per week working fulltime during the school year:

Monday-Friday 8am to 430pm with variation of summers off being paid for 32 hours/week year-round.

Private vehicle will be used to travel to patient’s school to provide Asthma related care/education to the patient and school staff. Valid Ohio driver’s license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines.

Job Description Summary:

Assesses, plans, implements, facilitates, and evaluates educational programming, orientation, skill development activities, and continuing education. Assists in carrying out appropriate plan of care for patients with chronic disease. Conducts visits in area schools and travels to schools.

Essential Functions:

  • Performs chronic disease follow-up visits at schools according to algorithm of care and communicates related to child’s medication regime, identifies needs for home and school medications reconciliation, assists to obtain prior authorizations, maintains records, initiates changes, and transports medications. Functions within the nursing practice guideline and performs all patient management activities along with the collaborating APN/physician.
  • Provides family-centered care to meet other family needs such as psycho-social support, education, and collaboration in discharge planning. Obtains appropriate consents to participate, assess, and discuss plan of care with the family, communicates with school nurses, Primary Care Providers, and specialty providers.
  • Collaborates in recruitment and implementation of programs at new schools, acting as clinical resource. Maintains clinical competency and stays informed of current trends. Participates in planning and presenting educational programs. Plans and participates in the education of RN, school nurses, and staff new to the school program.
  • Remains informed of current research and continues to look for evidence-based practice, making data available to staff and applying it as appropriate. Initiates and participates in education, research, professional, and organized activities that contribute to improving patient care, the scientific knowledge base of nursing, and program operation.
  • Performs diagnostic and therapeutic procedures based in specialty area according to established guidelines, as necessary.
  • Establishes professional objectives, evaluates self, and shares progress periodically with program APN and the Physician Lead. Maintains involvement with professional nursing organizations to promote collegiality and networking. Assists in educating others regarding the role of the nurse educator. Mentors regarding clinical competence and professional development.

Education Requirement:

  • Bachelor of Science in Nursing, required.
  • Master’s degree, preferred.

Licensure Requirement:

  • Registered Nurse with current licensure to practice nursing in the State of Ohio, required.

Certifications:

Basic-Life Support (BLS) certificate, required.

  • Demonstrated nursing competence with specific clinical skills and knowledge in disease management.
  • Demonstrated leadership, interpersonal skills, teaching ability, advanced level of critical thinking, organizational, and decision-making skills.
  • Excellent verbal and written communication skills.
  • Computer skills to maintain program and patient data.

Experience:

  • 4 years of nursing experience, required.
  • Pediatric setting experience, preferred.
