What You’ll Do
- Maintain up-to-date consumer treatment plan records in eCare, including PCP Team packets, treatment plan addendums, and demographic updates. Ensure current and accurate demographic information is provided to the appropriate HNBHS administrative staff.
- Assist Case Managers and Program Directors in requesting, tracking, and updating all required treatment consents and related correspondence. Complete and organize all documentation in eCare as directed.
- Serve as a liaison for external providers, including guardians and county Case Managers, in the absence of internal Case Managers or Program Directors.
- Assist consumers when they work at the vocational program. Many of our individuals need guidance as they work and she would be responsible for ensuring that they develop skills to eventually work independently and ensuring that they are performing their job task appropriately.
- Participate in consumer intake assessments and complete admission documentation as needed. Support the development of initial treatment plans and coordinate intake arrangements to align with individual consumer needs.
- Attend Person-Centered Planning (PCP) Team meetings as a residential representative when requested by the treatment team, Case Manager, or Program Director.
- Participate in daily and weekly Treatment Team meetings; accurately document and distribute meeting notes in a timely manner.
- Assist with annual documentation processes and data collection, while maintaining consumer records in compliance with HNBHS and regulatory standards; conduct periodic record audits as directed.
- Adhere to licensing and accreditation standards for assigned programs.
- Support Case Managers in networking and collaborating with external agencies and service providers.
- Follow general safety procedures and report any unsafe conditions to the appropriate manager.
- Perform general office duties, including typing, filing, scanning, uploading documents to resident charts, and sorting mail.
Work Schedule
On-site | Monday–Friday, 8:00 AM – 4:00 PM
Qualifications
- High School Diploma or equivalent required; Associate’s Degree preferred.
- Experience working with individuals with mental health, physical, emotional, social, or vocational needs
- High level of professionalism, confidentiality, and integrity (HIPAA compliance required)
- Ability to provide routine and emergency communication to individuals, families, and professionals
