Under the direction of the VP, the Director plans, implements and directs population health programs across SBH Health System. This includes operational leadership, direction, and oversight of the Routine HIV Screening program across Emergency, Inpatient, and Ambulatory Care settings. The role directs clinical workflow development, Epic EMR enhancements, workforce training, quality improvement, and data reporting. As a member of the department’s leadership team, the position holder collaborates with stakeholders to drive program excellence and aligns initiatives with SBH health system's goals, promoting innovative and effective patient-centered care.
- Provides leadership to the Ambulatory Care Quality & Population Health department, supporting the VP with department strategy, operations and management.
- Directs the development and implementation of population health programs to improve patient care and operational performance.
- Directs performance improvement projects for the SBH Ambulatory Care Quality Assurance and Performance Improvement (QAPI) Committee.
- Collaborates with clinical and administrative leaders to direct clinical workflow development, Epic EMR enhancements, workforce training, quality improvement, and data reporting
- Directs the development of staff training content in partnership with HR and clinical leaders.
- Participates in the departmental leadership team, leading change across the department and its divisions.
- Supports the Vice President in implementing new clinical workflows and billable services, ensuring growth in services, volumes and revenues.
- Collaborates with clinical and administrative leaders to identify growth opportunities and researches and develops proposals & applications.
- Manages and supervises assigned employees / teams, including work assignments, attendance, discipline, performance evaluations and planning requirements.
- Performs other related duties at the discretion of the Vice President.
