The registered nurse (RN) evaluates, defines, coordinates and administers the total nursing care of an assigned group of patients. The nurse writes the Plan of Care (POC) in tandem with the patient according to the patient's goals and the provider ordered primary need for home health nursing services. The nurse is responsible for admission, recertification and discharge visits including OASIS dataset collection and timely submission. The RN is the home health case manager for nursing patients and will advocate for, and coordinate care between all health care team members, the patient, and patient family/caregiver. The nurse is responsible for the direction and supervision of any personnel assigned to them and will complete supervisory visits as assigned. The RN informs the provider and other personnel of changes in the patient's condition and needs and educates and supports the patient in meeting care goals. The nurse will recommend and refer to outside resources as needed. The RN will need to be competent to care for patients of all ages and demographics, throughout the continuum of care from the time of initial referral, through discharge from agency. Other clinical duties will be assigned during periods of low patient census. The RN is required to participate in the on-call rotation including nights, weekends and holidays.

Required – Graduate of an accredited school of nursing

Required - Current Oregon RN license without restrictions

Required – Current BLS certification

Required – Current Oregon driver’s license in good standing

Required – Personal vehicle in reliable condition with proof of auto insurance

Preferred – 2 year experience as an RN in an acute healthcare setting

Preferred – 1 year experience as an RN in a community health setting

Coordination of interdisciplinary services for patients. Coordination and scheduling of patient visits to ensure productivity standards are upheld. Participates in nursing on-call schedule. Position includes clinical office duties as assigned. Ability to lift 75 pounds max with frequent carrying/lifting of up to 50 pounds. Able to sit, stand, walk, bend, squat, reach, and stretch frequently. Ability to communicate effectively through reading, writing, and speaking in person or telephone. Frequent computer use, must demonstrate manual dexterity. Occasional exposure to blood, body fluids or tissue, communicable diseases, chemicals, including but not limited to sticks or cuts by needles and other sharp items. Able to perform primary duties and responsibilities of the job with or without reasonable accommodation.

QUALIFICATIONS:

  • Graduate of an accredited school of Nursing required.
  • Current Oregon RN license without restrictions required.
  • Evidence of continuing education appropriate for skill level and area of practice preferred.
  • Current CPR Certification required.

Work experience:

  • Minimum of 1-year experience as a professional nurse in the clinical area required.
  • Minimum of 1-year experience in a community health setting preferred.

Skills & Competencies:

  • Able to prepare and maintain patient records and reports.
  • Must possess quality writing and verbal skills.
  • Ability to establish and maintain effective working relationships with patients, caregivers, facility staff, the patient’s family, peers, physicians, and ancillary department staff.
  • Able to react calmly and effectively in urgent and emergent situations.
  • Able to identify problems and recommend solutions.
  • Must demonstrate strong personal integrity, stamina, and emotional stability.
  • Share responsibility for 24-hour on-call on a rotational basis with other Home Care Services nursing staff.
  • Ability to communicate therapeutically.
  • Possess the ability to provide care for the patients’ age-specific, cultural, physical and psychosocial well-being.

ESSENTIAL JOB FUNCTIONS:

  • Have a vehicle in reliable condition with insurance coverage and a current Oregon driver’s license.
  • Ability to lift 50 pounds’ maximum with frequent lifting/or carrying of objects weighing up to 25 pounds.
  • Ability to drive a vehicle in all types of weather
  • Ability to withstand extreme seasonal temperatures
  • Able to complete patient visits efficiently, meeting required productivity standards.
