The Social Worker MSW will:

  • Identify patients and provide clinical social work, and behavioral health services:
  • Identify patients and conduct discharge planning evaluation and coordination of care which will include:
  • Assess the needs of patients referred/identified as appropriate to the ages of the patients and to the developmental life cycle stage (Neonates, Pediatrics, Adolescents, Adults, Geriatrics).
  • Formulate and implement the psychosocial and/or care coordination/discharge planning care plan for identified patients as appropriate to the ages of the patients and to the developmental life cycle stage in conjunction with the medical and interdisciplinary team and the patient/family.
  • Function as an interdisciplinary team member in a specialty area.
  • Document social work discharge planning activity in the patient's chart in accordance with Departmental, Hospital, CMS, QIO, DHHS and JC standards.
  • Social Work Transitions in Care functions:
  • Prepare monthly statistical and other assigned reports.
  • Develop and maintain community relations.
  • Participate in professional staff development and Hospital/Community activities to maintain skills and enhance professional visibility.

Requirements:

  • MSW from accredited Graduate School of Social Work. NJ Social Work License: LCSW or LSW.
  • NPI and Medical Non Billing Provider Number are required; the application should be submitted to the State within the first 30 days of employment.
  • Previous hospital experience preferred.
  • Experience in medical social work/perinatal pediatrics/behavioral health/case management preferred.
  • Position includes some weekend and holiday coverage.
  • EARC-PAS certified within 3 months of hire when applicable to assignment.

Salary Range: 65,229.00 - 104,354.00 USD

We offer competitive base rates that are determined by many factors, including job-related work experience, internal equity, and industry-specific market data. In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials.

The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
